螺内酯、速尿、开搏通联合治疗难治性心力衰竭66例临床观察

目的 采用自身对照试验研究方法观察螺肉酯、速尿、开搏通联合治疗难治性心衰的临床疗效。方法 对66例患者给予螺内酯100mg每日上午6时给予一次口服;速尿60mg每8小时一次静脉注射;开搏通25mg口服每日三次,如无副作用逐渐增至最大剂量50mg一次。连用14天为一疗程。每日观察休息状态下的心率、血压各4次,疗程前后做彩色心脏超声测定SV、CI、EF。结果 临床显效率72.7％,有效率19.7％,总有效率92.4％,治疗前后血压、SV、CI、EF均有显著差异(P<0.05)。结论 螺内酯、速尿、开搏通联合治疗难治性心衰是一种安全有效措施,用法简便,尤其适合基层医院推广应用。

Clinical observtion on the effects of spironolactone,furosemide and captoril in the treatment of 66 cases of refractory heart failure

Objective To observe the clinical effects of combined treatment with spironolactone,furosemide and capto-ril in the patients with refractory heart failure. Methods 66 cases of refractory heart failure were treated with spironolac-tone 100mg. po. once daily ( 6AM ), furosemide 60mg. i. v. once every 8 hours and captoril 25mg-50mg. po. three times daily, for 14 days. The heart rate blood pressure (BP),stroke volume (SV) cardiacindex(CI) and ejection fraction(EF) of patients wese measused with UCG before and after the treatment. Results The clinical total effective rate was 92. 4%. All the BP. SV. CI and EF were significantly improved (P < 0. 05). Conclusion The method of combined treatmint with spironolactone, furosemide and captoril is safe and effective for the patients with refractory heart failure. It is especially suitable to adopt in local hospital.
