TURP、TVP与耻骨上前列腺切除对术后患者性功能的影响比较

目的 比较经尿道前列腺电切术（TURP）、经尿道前列腺电汽化术（TVP）和耻骨上前列腺摘除术（SPP）治疗前列腺增生症对术后患者性功能的影响。方法 对SPP40例、TURP25例和TVP25例术后进行3个月和12个月的追踪观察，总结勃起功能障碍（ED）和逆行射精的发生率。结果 TVP、TURP和SPP组术后12个月ED发生率分别为8％（2／25）、36％（9／25）和40％（16／40），逆行射精发生率分别为44％（11／25）、48％（12／25）和42．5％（17／40）。结论 三种方法治疗BPH导致术后勃起功能障碍TVP优于其它两种术式（P〈0．05），导致逆行射精则无统计学意义。

Comparision of postoperative sexual function following different approaches of prostatic surgery on benign prostatic hyperplasia

Objective:To observe the effects of transurethral vaporization for prostate(TVP) and suprapubic prostatectomy(spp) and transurethral resection of the prostate(TURP) on sexual function of the patients with benign prostatic hyperplasia(BPH).Methods:Following up for 6 and 12 months,the incidences of erectile dysfunction(ED) and retrograde ejaculation in 40 cases treated with SPP were compared between operation(TURP and TVP in 25 cases respectively).Results:The incidences of ED were 8%(2/25),36%(9/25)and 40%(16/40)in those cases treated with TVP,TURP and SPP.The incidents of erectile dysfunction were 44%(11/25),48%(12/25)and 42.5%(17/40)respectively.Conclusions:In a view of ED on after surgical treatment of BPH,TVP is superior to TURP and SPP.There is no obvious difference among TVP,TURP and SPP on the incidence of intrograde ejeculation.
