65例高血压病患者的心率变异性分析

目的：分析高血压病患者心率变异性（ＨＲＶ），了解心血管自主神经功能（ＣＡＦ）的变化。方法：应用２４ｈ动态心电图检测６５例高血压病患者及４５例健康者的ＨＲＶ，并将高血压病按靶器官损害程度及血压昼夜节律情况分别分为２组，对ＨＲＶ进一步分析。结果：（１）高血压病组ＨＲＶ指标明显低于健康组。（２）高血压病Ⅱ—Ⅲ期组ＨＲＶ指标明显低于Ⅰ期组及健康组。（３）高血压病动态血压呈非勺型组ＨＲＶ指标明显低于勺型组及健康组。结论：高血压病ＨＲＶ指标降低，心血管自主神经功能失衡且与靶器官损害程度呈正相关。ＨＲＶ的降低与血压昼夜节律的消失有关系。ＨＲＶ可作为评价高血压病心血管自主神经功能的一种手段，也是高血压病预后的一个预报因子。

Analysis of Changes in Heart Rate Variability in 65 Patients with Hypertension

bjective: The purpose of the study was to investigate the characteristic of heart rate variability (HRV) and cardiovascular autonomic function (CAF) in 65 hypertensive patients. Methods: HRV was evaluated in patients with essential hypertension and 45 control subjects using 24hours ambulatory ECG(Holter) recording. According to the degree of target organs damage and different circadian blood pressure rhythmicity,the patients with hypertension were divided into two groups. Results: (1) Compared with the control group,the indices of HRV were markedly reduced in hypertension group.(2)Compared with the control group and the first stage hypertension group, the indices of HRV were markedly reduced in the second and the third stage hypertension groups.(3)Compared with the control group and the dipper type hypertension group,the indices of HRV were markedly reduced in the nondipper type hypertension groups. 5HZConclusion: HRV indices were decreased and AFC was imbalanced correlating positively to the degree of target organs damage in hypertensive patients.The decreased HRV was associated with the disappearance of circadian blood pressure rhythmicity. HRV could be used as a good method to evaluate CAF in hypertensive patients and a forecast factor to the prognosis of hypertension.
