胸腔出入口区CT解剖及其临床应用评价

目的探讨胸腔出入口区的正常CT解剖和CT对该区病变的诊断及鉴别诊断价值。方法回顾性分析经病理及临床证实的127例胸腔出入口区病变的CT表现,对60例正常对照组在CT上分为5个解剖区:1区(甲状腺区);2区(甲状腺内侧区);3区(甲状腺外侧区);4区(甲状腺后区);5区(甲状腺前区)。结果在127例中,病变位于1区18例(12.40%),2区8例(6.84%),3区75例(61.54%),4区1例(0.85%),5区3例(2.56%),跨1、3、5区15例(12.8%),跨1、2、3区7例(5.98%)。病变主要分布在3区,其中以淋巴结病变最为多见(60例);甲状腺病变(33例)主要分布在1区和跨区生长。结论在CT上对胸腔出入口解剖区域的合理分区,有利于病变的定位和定性诊断,具有重要的临床应用价值。

Evaluation in Clinical Application and Anatomy of the Thoracic Outlet and Inlet with CT

Objective To study CT anatomy and diagnostic value of the thoracic outlet and inlet.Methods CT features of the lesions in the thoracic outlet and inlet proved pathologically and clinically in 127 cases were analyzed retrospectively.Control group included 60 normal cases,the regions of thoracic outlet and inlet were divided into 5 anatomic regions:1 region(thyroid zone),2 region(interior thyroid),3 region(Lateral thyroid),4 region(posterior thyroid),5 region(anterior thyroid).Results In 127 cases,the lesions localized in 1 region in 18 cases(12.40%),2 region in 8 cases(6.84%),3 region in 75 cases(61.54%),4 region in 1 case(0.85%),5 region in 3 cases(2.56%),transregional 1,3,5 region in 15 cases(12.8%)and transregional 1,2,3 region in 7 cases(5.98%).The disorders were mainly distributed in 3 region in which lymphatic node disorder was the most common(60 cases).The thyroid gland lesions were mainly in 1 zone and involved with other zones.Conclusion On CT,the anatomic regions of the thoracic outlet and inlet are divided into 5 zones,which are of very important value in diagnosis of disorders in these regions.
