糖尿病健康教育模式的应用和前景展望

摘    要:广大糖尿病教育者在进行健康教育时融合了全程教育模式、护理程序模式、基于问题的教育模式、临床路径模式、赋能教育模式等多种教育模式.每个模式均有各自的侧重点,如全程教育模式强调门诊、院内、院后随访教育一体化;护理程序模式强调教育前应对患者进行评估,再制定教育计划,教育后需进行教育效果的反馈;赋能教育模式强调教育者为患者提供信息和支持,鼓励患者自己作出糖尿病自我管理的决定.目前教育模式的发展越来越趋向于更加注重以患者为中心,且强调行为改变和患者自我管理能力的提高.

Abstract:Diabetic educators have explored a lot of different educational models,such as whole procedure education model, nursing process model, problem-based learning education model, clinical pathway model,the theory of empowerment education,ect. Every model has different emphatic aspects. Whole procedure education model emphasized outpatients, inpatients and follow-up education integrated. Nursing process model stressed patients' assessment before education,development of educational program,feedback of evaluation. Empowerment education underlined that patient was viewed as problem solver and caregiver,and professional acted as a resource and helped the patient to set goals and develop a self-management plan. Now a lot of educators have been paying more attention to patient-centered nursing,focusing on behavioral change, and stressing the self-management of patient.
