小儿先心病心内直视术后早期死亡原因分析及对策

本文对713例小儿先心病心内直视术后早期死亡的原因进行了回顾性分析。术后死亡37例。低心排血量综合征、呼吸窘迫综合征和严重室性心律失常是导致术后早期死亡的主要原因。作者就防止或减少这类手术合并症的处理对策进行了讨论。

Analysis of the Causes of Early Postoperative Deaths and Its Appropriate Measures Following Open Heart Surgery in Children with Congenital Heart Disease

A series of 713 cases of congenital heart disease under going open heart surgery were studied retrospectively. There were 37 early postoperative deaths. The most common causes were low cardiac output syndrome respiratory distress syndrome and ventricular arrhythmia. The complexity or unsatisfactory correction of the cardiac anomalies, severity of the pulmonary hypertension and ischemic myocardial injury during open heart surgery were the main factors determining the incidence of low cardiac output syndrome. The causes of respiratory distress syndrome were closely related with the pulmonary hypertension or perfusional lung oeeurring in the process of cardiopulmonary bypass. Minimization of ischemic myocardial injury during open heart operation might be considered as an effective way to prevent the occurrence of ventricular arrhythmias.
