经颅彩色多普勒超声诊断颅内段椎-基底动脉狭窄的应用价值

摘    要:目的探讨经颅彩色多普勒血流显像(TCDFI)诊断颅内段椎-基底动脉狭窄(VBS)的艋床价值。方法选择颅内段VBS所致的短暂性脑缺血发作患者40例,用二维超声探头经枕窗探查颅内段椎-基底动脉(VBA),观察血管充盈情况及血流动力学变化,部分较重者进行数字减影血管造影(DsA)检查对比。结果病例组和对照组血流充盈情况及频谱形态明显不同,收缩期峰值流速(PSV)和搏动指数(PI)差异具有显著性(P〈0.05或P〈0.01)。并得出TCDFI诊断VBS的敏感性为85.7%,特异性为83.3%。结论TCDFI能清晰、直观地检出颅内VBA有无狭窄及其程度,提供有价值的血流动力学参数,作为筛选VBS的首选方法。

Abstract:Objective The aims of this study were to discuss clinical value of TCDFI in diagnosing vertebro-basilar stenosis(VBS).Methods Forty patients with TIA were studied in the patient group.The intracranial VBA was scanned through sub-occipital window by TCDFI.The vessels and changes of hemodynamical parameters were observed.Some cases with serious VBS were confirmed with DSA.Results There were significant differences in the color flow and spectrum between the controls and the patient groups and the differences were significant(P<0.05 or P<0.01) in the hemodynamical parameters of PSV and PI between patients and healthy subjects.The sensitivity was 85.7% and the specialty 83.3% in detecting VBS by TCDFI.Conclusions Intracranial VBA can be detected clearly and directly by TCDFI and the hemodynamics can be assessed successfully.TCDFI is a preferred investigation on the patients suspected of VBS.
