CT联合超声与CA125检测在卵巢癌诊断中的应用价值

目的探讨CT联合超声、CA125检测对卵巢癌的诊断价值。方法回顾分析311例经超声检查、CA125检测及CT检查的卵巢肿瘤患者,以术后病理诊断标准分析三者的诊断价值。结果 311例患者中CT诊断良性肿瘤1例（2.56%）,恶性36例（92.3%）;超声诊断良性肿瘤163例（52.4%）,恶性112例（36.0%）;两者联合CA125良性肿瘤1例（2.56%）,恶性38例（97.4%）;经超声诊断卵巢癌的灵敏度为86.8%,特异度为71.4%,准确度为88.4%,约登指数为58.2%;经CA125诊断卵巢癌的灵敏度为54.2%,特异度为84.1%,准确度为71.7%,约登指数为38.3%;经CT诊断卵巢癌的灵敏度为94.6%,特异度为50.0%,准确度为92.3%,约登指数为44.6%;CT联合超声、CA125诊断卵巢癌的灵敏度为97.4%,特异度为50.0%,准确度为97.4%,约登指数为94.8%;三者联合检查比单一检查诊断卵巢癌准确性较高,差异有统计学意义（P〈0.05）;CT检查的卵巢癌患者术前分期与术后病理分期比较,差异无统计学意义（P〉0.05）。结论术前CT联合超声、CA125检测可以提高卵巢癌的诊断准确性,CT在卵巢癌分期诊断中有较高的准确性。

CT joint ultrasonic and CA125 testing on diagnosis value of ovarian cancer

Objective to discussion the value of CT combined with ultrasound,CA125 test for ovarian cancer diagnosis.CT in the staging of ovarian cancer of value.Methods 311 cases cancer patients were decete by CA125,CT and Ultrasound.Results 1 case（2.56%） benign tumor and 36 cases（92.3%） malignant tumor were diagnosed by CT.163 cases（52.4%） benign tumor and 112 cases（36.0%） malignant tumor were diagnosed by Ultrasound.1 case（2.56%） benign tumor and 38 cases（97.4%） malignant tumor were diagnosed by CT,Ultrasound joint CA125.The sensitivity of ultrasound diagnosis of ovarian cancer was 86.8%,specificity was 71.4%,accuracy was 88.4%,Youden index was 58.2%;The sensitivity of CA125 of the diagnosis of ovarian cancer was 54.2%,the specificity was 84.1%,the accuracy was 71.7%,Youden index was 38.3%;the sensitivity of CT diagnosis of ovarian cancer was 94.6%,specificity was 50.0%,accuracy was 92.3%,Youden index was 44.6%;the sensitivity of CT combined with ultrasound,CA125 ovarian cancer was 97.4%,the specificity 50.0%;the accuracy was 97.4%;Youden index was 94.8%;Three of the Joint Inspection diagnosis of ovarian cancer than a single high accuracy,statistical analysis were significantly different（P〈0.05）.Conclusion Pre-operative CT combined with ultrasound,CA125 test can improve the accuracy of diagnosis of ovarian cancer.CT in the staging of ovarian cancer have a higher accuracy.
