脾蒂结扎法在腹腔镜脾切除术中的应用

目的探讨脾蒂结扎法在腹腔镜脾切除术（LS）中的应用价值及临床疗效。方法 2009年1月至2011年3月对8例难治性特发性血小板减少性紫癜和4例遗传性球形红细胞增多症（其中3例合并胆囊结石）患者,采用腹腔镜下超声刀离断脾周韧带,丝线结扎并钛夹夹闭脾蒂,切除脾脏。合并胆囊结石者,同时切除胆囊。结果 12例均完成腹腔镜脾切除术,平均手术时间102 m in,术中出血平均90 ml,无中转开腹,术后2例切口皮下血肿。结论脾蒂结扎法行腹腔镜脾切除术,操作简单,安全可靠,手术费用低,是最易推广应用的手术方法。
