血液透析患者超敏C反应蛋白与心脏瓣膜钙化的相关性研究

摘    要:目的探讨维持性血液透析(MHD)患者超敏C反应蛋白(hs-CRP)与心脏瓣膜钙化(VC)的相关性及其临床意义。方法选择2011年1月至2012年12月淮安市第一人民医院分院收治的34例MHD患者作为观察组,另外同期随机选择35例健康体检者作为对照组,测定两组受试者的血清hs-CRP水平,并检测血清总胆固醇(TCH)、三酰甘油(TG)、低密度脂蛋白(LDL)、血清总蛋白(TP)、白蛋白、血钙、血磷、甲状旁腺激素(PTH)水平。同时应用彩色B超检测心脏VC情况,并根据心脏彩超检查有无VC分为心脏VC组和无心脏VC组,比较两组实验室检查结果。结果 MHD患者hs-CRP水平及心脏VC发生率均高于对照组(P<0.05);心脏VC组与无心脏VC组患者的年龄、透析时间、白蛋白、PTH、LDL、血钙、血磷、hs-CRP水平比较差异均有统计学意义(P<0.05)。结论血清hs-CRP水平与心脏VC关系密切,MHD患者心脏VC的发生是多种因素综合作用的结果,hs-CRP水平对MHD患者的心血管事件发生具有重要意义。

Abstract:Objective To discuss the correlation between high-sensitivity C-reactive protein (hs-CRP) and heart valve calcification( VC ) in maintenance hemodialysis(MHD) patients and its clinical significance. Methods A total of 34 MHD patients and 35 healthy controls from the First People' s Hospital of Huai'an City were enrolled in this study from Jan. 2011 to Dec. 2012. Levels of serum hs-CRP, total cholesterol( TCH), triglyceride ( TG), low density lipoprotein ( LDL), total protein ( TP), albumin(Alb), calcium(Ca) ,phosphorus (P), parathyroid hormone (PTH), and so on, were detected. And conditions of heart VC were checked up by means of color Doppler ultrasound, and the patients were divided into VC group and non-VC group, the laboratory test results were compared. Results MHD patients hs-CRP levels and cardiac VC were higher than the controls(P 〈 0.05 ) ; patients age, duration of dialysis, Alb, PTH, LDL, Ca, P, hs-CRP level differences between VC group and non-VC group were statistically significant(P 〈 0.05 ). Conclusion There is a close relationship between serum hs-CRP and heart VC ,and the heart VC is the result of comprehensive factors in MHD patients. Serum hs-CRP level is of great significance to predict the events of cardiovascular system and death in MHD patients.
