2型糖尿病患者亚临床甲状腺功能减退症与血管并发症关系的研究

目的 探讨2型糖尿病患者亚临床甲状腺功能减退症（甲减）与血管并发症之间的关系.方法 将2009年1月至2012年12月在盐城市亭湖区人民医院内分泌科住院治疗的糖尿病患者275例,按照是否合并亚临床甲减分为合并亚临床甲减组和单纯糖尿病组.对比分析两组患者血管并发症状况及其临床特征.结果 2型糖尿病合并亚临床甲减组女性所占比例（64.58%）显著高于单纯糖尿病组（46.70%）,病程（9.3±1.7）年显著高于单纯糖尿病组（8.3±1.6）年,胆固醇（5.2±0.9） mmol/L]和三酰甘油（1.9±0.9） mmol/L]水平也分别高于单纯糖尿病组（4.8±1.0） mmol/L、（1.5±0.8） mmol/L],两组差异均有统计学意义（P〈0.05）.合并亚临床甲减患者糖尿病肾病和心血管疾病的发生率分别为64.58%和20.83%,均高于单纯糖尿病组的45.82%和8.81%,且差异有统计学意义（P〈0.05）.结论 2型糖尿病合并亚临床甲减者糖尿病肾病和心血管疾病并发症的发生率也高于无亚临床甲减者,应定期检查,以及时发现,及时治疗.

The Correlation between the Symptoms of Hypothyroidism and Vascular Complications in Type 2 Diabetes Patients

Objective To investigate the relation between the symptoms of hypothyroidism and vascular complications in type 2 diabetes patients. Methods. During Jan. 2009 and Dec. 2012,275 cases of diabetes treated by endocrine department of People＇s Hospital of Yancheng City were divided into combined with sub- clinical hypothyroidism group and the diabetes alone group according. The clinical features and vascular complications of the two groups were compared. Resnlts The female proportion （64.58 % ）, course of the disease（9.3 ± 1.7 ） （ year）, Total cholesterol （ 5.2 ± 0.9 ） mmol/L and Triglyceride levels （ 1.9 ± 0.9 ） mmol/L in type 2 diabetes combined with subclinical hypothyroidism were significantly higher than that of the diabetes alone group （46.70%） ; （ 8.3 ± 1.6 ） year; （4.8 ± 1.0 ） mmol/L, （ 1.5 ± 0.8 ） mmol/L （ P 〈 0.05 ）. The incidence of diabetic nephropathy and cardiovascular disease in combined hypothyroidism patients were 64.58% and 20.83% ,higher than that of the diabetes alone group （45.82% ,8.81% ） ,and the difference was statistically significant （ P 〈 0.05 ）. Conclusion The incidence of diabetic nephropathy and cardiovascular disease in combined hypothyroidism patients were higher than that of the diabetes alone group. It should be checked regularly, and treated timely.
