难治性突发性耳聋的相关因素分析及中西医结合治疗

目的分析难治性突发性耳聋的相关致病因素及中西医结合治疗的效果。方法回顾性分析2006年1月至2009年1月孝感市中心医院收治的260例突发性耳聋患者的临床资料,将其按照不同的治疗方法分为两组：西医治疗组（130例）应用西医药物和激素治疗,中西医结合组（130例）在西医的基础上应用中医结合治疗。分析难治突发性耳聋的相关因素,观察并比较两组患者的治疗效果及治疗后的生活质量。结果多因素Logistic回归分析结果显示,患者的年龄与发病及临床疗效关联较大,且年龄越小,发病后越早治疗,疗效越好（P〈0.05）。中西医结合治疗组总有效率高于西医治疗组（93.08%vs 84.62%）,差异有统计学意义（u=4.284,P〈0.05）。中西医结合治疗组患者的总体健康评分、积极感受评分、日常生活能力评分、性生活评分均高于西医治疗组（P〈0.05）,疼痛与不适评分、依赖性评分低于对照组（P〈0.05）。结论中西医结合治疗突发性耳聋患者,标本兼治,疗效显著,患者生活治疗明显提高。

Analysis of Related Factors for Refractory Sudden Deafness and the Combined Traditional Chinese and Western Medicine Treatment

Objective To analyze the intractable sudden deafness related pathogenic factors and the effect of combining traditional Chinese and western medicine treatment. Methods A retrospective analysis on the clinical data of 260 cases of sudden deafness during Jan. 2006 and Jan. 2009 admitted by Xiaogao Center Hospital was done,which were divided into western medicine group（ 130 cases） and combined treatment group（ 130 cases）. The western medicine group was treated with western drugs and hormone,the combined treatment group was also given traditional Chinese medicine based on the western medicine treatment. The refractory sudden deafness related factors were analyzed,and the therapeutic effect and quality of life after treatment of the two groups were observed and compared. Results Multivariate Logistic regression analysis showed that the patient＇s age and the incidence and clinical effect were more correlated,the younger the onset age and the sooner the treatment,the better the effect（ P 0. 05）. Combined treatment group total effective rate was higher than western medicine treatment group（ 93. 08% vs 84. 62%）,the difference was statistically significant（ u = 4. 284,P 0. 05）. The overall health score,positive feelings score,ADL score, sex life scores of the combined treatment group were higher than western medicine group（ P 0. 05）; pain and discomfort scores,dependence score lower than the western medicine group（ P 0. 05）. Conclusion Combined traditional Chinese and western medicine treatment for sudden deafness has the advantages of treating both symptoms and causes of the disease with significant efficacy,improving quality of life the patients considerably.
