类风湿关节炎合并下肢溃疡6例患者的临床观察

目的 讨论类风湿关节炎（RA）合并下肢溃疡的临床表现、治疗及预后.方法 回顾性分析2007年4月至2012年1月在苏州市中医医院风湿科就诊的6例合并下肢溃疡的RA患者的临床症状、实验室指标及治疗情况.结果 经治疗后,所有患者的溃疡愈合,关节症状缓解,实验室指标类风湿因子、C反应蛋白、红细胞沉降率较治疗前显著下降.患者治疗时间在1个月至1年之间,平均4月,随访6个月后未再次出现局部溃疡.结论 RA合并下肢溃疡患者病情较重,应早期治疗,激素和环磷酰胺等免疫抑制剂的早期联合应用可有效改善症状.

Clinical Observation of Six Rheumatoid Arthritis Patients with Lower Limb Ulcer

Objective To investigate the clinical manifestation, treatment and prognosis of rheumatoid arthritis（RA） with lower limb ulcers. Methods The clinical data of six patients treated by Suzhou Chinese Medicine Hospital during Apr. 2.007 and Jan. 2012 were retrospectively analyzed by clinical symptoms, laborato- ry parameters and treatment effects. Results All of the six patients were observed improvement in the joint symptoms,ulcer healing,and index of RF,CRP,and ESR. Patient treatment time was between 1 month and 1 year, an average of 4 months ,after 6 months of follow-up local ulcer did not occur again. Conclusion Lower limb ulcers is a serious condition of the patients with RA,which needs early treatment. The early application of immunosuppressive agents such as hormones and eyclophosphamide are effective in improving symptoms.
