辛伐他汀在改善高龄AECOPD患者肺功能中的临床应用效果观察

目的 探讨辛伐他汀在改善高龄慢性阻塞性肺疾病急性加重期（AECOPD）患者肺功能的临床效果.方法 将重庆市肿瘤医院2009年10月至2012年11月收治的72例AECOPD患者根据治疗方式不同分为两组：对照组32例,采取常规治疗;观察组40例,在对照组治疗的基础上加用辛伐他汀治疗,比较两组患者治疗1周后的第一秒用力呼气量占用力肺活量百分率（FEV1%）、FEV1/用力肺活量（FVC）（%）及诱导痰血管内皮生长因子的差异.结果 对照组患者治疗1周后FEV1%、FEV1/FVC及诱导痰血管内皮生长因子较治疗前无明显改变（P＞0.05）;观察组治疗1周后FEV1%、FEV1/FVC及诱导痰血管内皮生长因子较治疗前明显改善,较同期对照组患者也具有明显优势（P＜0.05）,且诱导痰血管内皮生长因子与FEV1%、FEV1/FVC呈正相关（r=0.436,0.368,P＜0.05）.结论 辛伐他汀可以有效地改善高龄AECOPD患者的肺功能,同时显著提高患者诱导痰血管内皮生长因子水平,提示患者肺功能改善与患者肺内血管恢复相关.

Clinical Application Effect of Simvastatin to Improve Lung Function of Elderly Patients with AECOPD

Objective To explore the clinical effect of simvastatin to improve lung function of patients with acute exacerbation of chronic obstructive pulmonary disease （AECOPD）. Methods Seventy-two AECOPD cases fi＇om Chongqing Cancer Hospital during October 2009 and November 2012 were divided into control group and observation group according to the different treatments ： the control group （ 32 cases） was given conventional treatment,the observation group（40 cases）was given simvastatin on the basis of the con- trol group therapy, FEV1 （forced one second expiratory volume）% , FEVI/FVC （forced vital capacity）% and vascular endothelial growth factor induced sputum differences of the two groups after one week of treatment were compared. Results After one week of treatment FEVI%, FEV1/FVC and induced sputum vascular endothelial growth t／actor of the control group were not significantly changed from before treatment （ P 〉 0.05 ） ;while that of the observation group improved significantly from before the treatment, which also had obvious advantages compared to the control group of the same period（ P 〈 0.05 ）, and induced sputum vascu- lar endothelial growth factor was positively correlated with FEVI % , FEVI/FVC （ r = 0. 436,0. 368, P 〈 0.05 ）. Conclusion Simvastatin can effectively improve the lung function of elderly patients with AECOPD, while significantly increasing the levels of induced sputum vascular endothelial growth factor, which suggests tha the improvement in lung function is associated with vascular recovery.
