螺旋CT在肺栓塞临床诊断中的应用

摘    要:目的探讨螺旋CT在肺栓塞临床诊断中的应用价值。方法随机选取2011年1月至2012年6月在佛山市禅城区张槎医院内科就诊的疑似肺栓塞患者42例,对患者分别进行普通CT-9螺旋CT检查后,比较两种方式的确诊率,同时观察患者CT图像。结果普通cT组共检测出16例肺栓塞患者,检出率为45.24%,螺旋cT组共检测出35例肺栓塞患者,检出率为88.10%。两组比较差异有统计学意义(P〈0.05)。普通cT和螺旋cT诊断肺栓塞的直接征象为肺动脉不同程度的充盈缺损,本次研究中,中心型充盈缺损(轨道征)普通CT检测出20支、螺旋cT检测出37支;部分充盈缺损普通cT检测出79支、螺旋cT检测出103支;完全阻塞普通cT检测出94支,螺旋cT检测出129支;附壁充盈缺损普通CT检测出68支、螺旋cT检测出97支,两组比较差异有统计学意义(P〈0.05)。结论螺旋CT可以在临床上用于诊断肺部栓塞,确诊率比普通CT高。

Abstract:Objeetive To discuss spiral. CT's application value in clinical diagnosis of pulmonary embolism. Methods During Jan. 2011 and Jun. 2012 from Foshan City Chancheng District Zhangcha Hospi- tal,42 cases with suspected pulmonary embolism were randomly chosen, after ordinary CT and spiral CT examinations respectively, diagnosis rate of the two methods were compared and CT imaging results were observed. Results In this study, 16 of them have been diagnosed out by ordinary CT,the detection rate was 45.24% ;and 35 cases of them diagnosed out by spiral CT,the detection rate was 88.10%. The difference between the two groups has statisticalsignificance( P 〈 0.05 ). Filling defect was the direct sign of pulmonary embolism by ordinary CT and spiral CT. Central type filling defect ( orbital syndrome) ordinary CT detected case was 20, spiral CT detected case was 37 ; partial filling defect ordinary CT detected case was 79, spiral CT detected case was 103 ; total occlusion ordinary CT detected case was 94, spiral CT detected case was 129 ; mural thrombus ordinary CT detected case was 68, spiral CT detected case was 97. The differences between these two groups had statistical significance( P 〈0.05 ). Conclusion Spiral CT can be adopted to diagnose pulmonary embolism in clinical, which has a higher definite diagnosis rate than ordinary CT.
