肥胖与瘦素对男童性发育影响的临床研究进展

人类瘦素水平与脂肪含量正相关,瘦素促进性发育和性成熟.肥胖男童由于存在瘦素抵抗,使瘦素对下丘脑-垂体-性腺轴作用减低,下丘脑促性腺激素释放激素分泌减少,同时瘦素对性器官的直接刺激作用减弱,可造成男童性发育迟缓或性腺发育不良;肥胖男童的高瘦素血症致使脂肪细胞或乳房上皮细胞的芳香化酶活力增强,雌性激素增多,故肥胖男童血清雌激素水平增高,出现女性化表现,如乳房肿大、腹部脂肪多等.该文就这方面近年来的发展作以综述.

Progress in clinical research on influence of obesity and leptin on sexual development of boys

It has been proved in many studies that level of leptin is positively correlated with fat composition,and leptin can promote sexual development and sexual maturation.However,those obese boys are leptin-resistant,which enables effects of leptin on hypothalamus-pituitary-gonadal axis decreased,so production of gonadotropin releasing hormone(GRH) is decreased,meanwhile,direct stimulation of leptin on sex organs is decreased and this results in delayed sexual development or hypoplasia of gonads of the boy.The hyperleptinemia of obese boys causes activity of aromatizing enzyme in adipose cells or mammary epithelial cells enhanced and estrogen level elevated.So the obese boy presents feminization,which in characterized by enlarged breast and thickened fat deposition in abdominal wall.In this article,we summarized progress in research on effects of obesity and leptin on sexual development of boys.
