激光冠状动脉成形术的临床应用

本文报告17例心肌梗塞及3例心绞痛患者在进行冠状动脉旁路移植术中((A)组,10例)和应用经皮穿刺血管方法((B)组,10例),采用Nd:YAG激光进行激光冠状动脉成形术。激光治疗血管共22支,其中左前降支12支,左旋支1支,对角支3支,钝园支1支,右冠状动脉5支。即刻效果,(A)组冠状动脉再通全部成功,1例患者冠状动脉激光部位有轻度血肿,(B)组病变部位狭窄程度由术前的90.1%降至术后的48.9%,仅有1例患者在术后加用气囊冠状动脉成形术时发生血管急性闭塞。本文对激光血管成形术的适应症、方法、远期疗效及并发症进行了探讨和研究。

CLINICAL APPLICATION OF CORONARY LASER ANGIOPLASTY

Nd: YAG Laser was used to perform coronary laser angioplasty (CLA) in 20 patients (including17 myocardial infarction and 3 angina pectoris). The patients were divided into two groups. Group A(10 patients) had CLA during coronary bypass operation, Group B (10 patients) had CLA duringpercutaneous transluminal angioplasty. There are 22 coronary arteries treated; 12 left anterior des-cending, 1 left circumflex, 3 diagonal, 1 marginal, and 5 right coronary arteries. The results showedthat in group A all vessels treated were successfully recanalized (a mild hematoma occurred in 1 case);in group B the percentage stenosis was reduced from 90.1% to 48.9%, An acute occlusion occurred in1 patient following balloon angioplasty. The application, methods, chronic effects and complications ofpercutaneous coronary laser angioplasty will be discussed in this report.
