脉冲式球囊扩张加服异烟肼治疗吻合口良性狭窄临床观察

目的探讨脉冲式球囊扩张加服异烟肼治疗对食管贲门癌术后吻合口良性狭窄的临床疗效。方法应用自制脉冲式油压器，哑铃型球囊，血管介入导丝及加强导丝，在500MAX线光机配备DSA监视系统下，对100例吻合口良性狭窄患者施行了扩张术。单纯扩张组50例，扩张后加服异烟肼组（联合组）50例。结果临床观察：两组同一时间段内随访，单纯组再狭窄复诊的几率相对较高，扩张次数增多；联合组较单纯组生活质量明显提高，全部病例扩张成功率达100％。结论脉冲式球囊扩张加服异烟肼后对吻合口良性狭窄的治疗效果满意，可有效抑制狭窄瘢痕的形成，疏松平滑肌组织，减少吻合口再狭窄的几率，生活质量明显改善，是一种操作比较简单，安全可靠，痛苦小，并发症少易接受的治疗新方法。

Clinical observation of pulse balloon dilatation and isoniazid for the treatment of benign anastomotic stenosis

Objective To investigate the clinical efficacy of pulse balloon dilatation and isoniazid in the treatment of benign anastomotic stenosis after esophageal and/or cardiac cancer resection. Methods With the help of home-made oil-pump, dumbbell-type balloon, vascular intervention wire and strengthening guide wire, benign anastomotic stenosis of 100 cases was dilated under 500 MA X-ray machine and its DSA surveillance systems. Of these cases, 50 received dilation only and the others were given isoniazid after dilation. Results The total success rate of dilation was 100%. All the patients were clinically followed-up at the same period. Restenosis and dilation frequency were relatively high in those undergoing dilation only. On the contrary, the life quality of the patients in combined treatment group was improved significantly. Conclu- sion Pulse balloon dilatation combined with isoniazid is effective in the treatment of benign anastomotic stenosis. It is a new, safe and reliable therapy which can effectively inhibit the formation of scar stenosis, loose smooth muscle, reduce the restenosis rate and improve the life quality, with characters of easy handling, little suffering and complication.
