泌乳素瘤血清泌乳素水平的相关影响因素

目的探讨泌乳素瘤患者血清泌乳素（PRL）水平升高的相关影响因素。方法泌乳素腺瘤61例,分析其术前泌乳素水平与年龄、性别、侵袭性、肿瘤大小、免疫组化PRL染色强度的关系。结果将所有病例按血清泌乳素水平分为≤100μg/L、〉100μg/L两组,进行二项分类logistic回归分析,入选变量为年龄、侵袭性、免疫组织化学PRL染色强度,OR值分别为0.933、12.034和2.666。结论泌乳素腺瘤患者血清泌乳素水平与年龄、肿瘤侵袭性和免疫组织化学PRL染色强度有关,但压迫丘脑和垂体柄导致泌乳素释放抑制因子释放减少有可能参与影响了泌乳素的释放。

The influential factors of serum prolactin level in prolactinoma

Objective To analyse the factors influencing elevation of serum prolactin（PRL） level in patients with prolactinoma. Methods Sixty-one cases of prolactinoma were studied. The relationships between preoperative prolactin level and age, gender,invasion,tumor size, and immunohistochemical activity of PRL were analyzed. Results The patients were divided into two groups ; the hormone level of group one was ≤100 μg/L,and that of the other group was 〉 100 μg/L. Binomial logistic regression analysis was carried out. The variables selected were age, invasion, and immunohistochemical activity of PRL. The OR of age was 0. 933 ,the OR of invasion was 12. 034 and the OR of immunohistoehemical activity of PRL was 2. 666. Conclusion Age, invasion, and immunohistochemical activity of PRL affect the serum prolactin level in prolactinoma patients, and the decrease in prolactin-inhibiting factor, as a result of oppression to thalamus and hypophysial stalk, may contribute to the release of prolactin.
