腹腔镜手术与开腹手术治疗早期子宫内膜癌的对比分析

摘    要:目的探讨腹腔镜手术用于治疗早期子宫内膜癌的价值。方法回顾性分析18例经腹腔镜手术和20例开腹手术治疗的临床I期子宫内膜癌患者的临床资料,比较两组的手术时间、术中出血量、并发症和术后恢复情况等。结果两组的手术成功率、手术时间、并发症、淋巴结切除数及术后恢复情况差异无显著性(P〉0.05)。术后随访均未有复发,无术后并发症。腹腔镜组术中出血量显著少于开腹组(P〈0.05),肛门排气时间及住院时间均显著短于开腹组(1.8±0.3对2.8±1.9;7.4±2.8对10.1±3.4)。结论从短期疗效看,腹腔镜手术可以完成广泛全子宫切除和淋巴结清扫,且术中出血少,术后恢复快,可作为早期子宫内膜癌手术治疗的方法之一。

Comparative analysis between laparoscopic and open approaches in treatment of early-stage endometrial carcinoma

Abstract:Objective To investigate the clinical value of laparoscopic operation in patients with early-stage endometrial carcinomas. Methods A retrospective analysis was performed to compare the clinical outcomes of laparoseopie and open surgery for stage I endometrial carcinoma in terms of operative time, blood loss,eomplieation,postoperative recovery and recurrence. Results All operations were performed with no events. The operation success rate, operative time, postoperative complication, number of lymph nodes excised and postoperative recovery were similar. The blood loss in laparoscopic group was significantly less than that in open group. Faster return of bowel function was observed in laparoscopic group ( 1.8± 0. 3 vs 2. 8 ± 1.9, P 〈 0. 05 ). The length of hospital stay in laparoscopic group was significantly shorter than that in open group(7.4 ± 2. 8 vs 10. 1 ± 3.4, P 〈 0. 05 ). Conclusion Laparoscopic radical hysterectomy and pelvic lymphadeneetomy can be used as an alternative method for the treatment of early-stage endometrial cancer.
