14例硼替佐米致心力衰竭文献分析

目的：分析硼替佐米致心力衰竭的发生规律及特点，为安全用药提供参考。方法：检索国内外硼替佐米致心力衰竭个案报道文献进行分析。结果：共检索到硼替佐米致心力衰竭个案报道11篇，涉及14例患者，男性8例，女性6例，年龄≥60岁的老年患者7例，有心血管疾病基础疾病及接受过蒽环类药物治疗分别有5例，有7例在第4次化疗周期后发生心力衰竭。硼替佐米致心力衰竭临床表现主要为呼吸困难，心胸比率增大，左、右室内径增加，LVEF下降，BNP增高。结论：有心血管疾病基础疾病及蒽环类药物使用史可能更易发生心力衰竭，应加强硼替佐米致心力衰竭监测，重点防治，减少其发生。

Analysis of bortezomib induced heart failure in 14 cases

OBJECTIVE To analyze the general patterns and characteristics of heart failure induced by bortezomib and provide a reference for clinical use.METHODS Cases of heart failure induced by bortezomib were collected and analyzed statistically in China and abroad.RESULTS A total of 11 reports involving 14 patients were collected, including 8 men and 6 women. Of these cases, 7 (50.0%) were older than 60 years, 5 had cardiovascular diseases and history of anthracycline therapy. Heart failure induced by bortezomib attacked after the fourth cycle of chemotherapy (n=7, 50.0%). The clinical manifestations of heart failure induced by bortezomib were dyspnea, increased heart-chest ratio, increased right and left ventricular diameters and decreased LVEF.CONCLUSION Failure induced by bortezomib may be related to cardiovascular diseases and history of anthracycline therapy. Clinicians should enhance monitoring on its use to detect and deal with heart failure.
