老年慢性心力衰竭及心肾综合征患者的临床特点及相关指标的研究

目的 探讨老年慢性心力衰竭及心肾综合征患者的临床特点及相关指标的变化情况,为临床诊治提供参考依据。方法 以宝鸡市第三人民医院2013-02～2016-02期间收治的150例心力衰竭患者作为对照组,将其设为观察组,将其中62例心肾综合征患者设为观察A组,88例非心肾综合征患者设为观察B组,同时另选80例同期进行健康体检者作为对照组,并比较各组各项指标变化情况。结果 观察组BNP前体、胱抑素C、尿素、肌酐水平均高于对照组(P＜0.05);观察A组BNP前体、胱抑素C、尿素、肌酐水平均高于对照组、观察B组(P＜0.05);观察B组BNP前体、胱抑素C水平高于对照组(P＜0.05),尿素、肌酐比较无显著差异(P＞0.05)。结论 老年慢性心力衰竭患者均伴有不同程度的肾功能损害,BNP与胱抑素C的敏感度高,前者可作为诊断心力衰竭的早期指标,后者可评估患者肾功能损害情况,对早期诊治心肾综合征具有重要意义。

Objective to investigate the clinical features and changes of related indexes in elderly patients with chronic heart failure

Objective To investigate the clinical features and changes of related indexes in elderly patients with chronic heart failure (CHF). Methods In 150 cases of heart failure patients admitted to our hospital from February 2013 to February 2016 as the research object,which will be set as the observation group,among which 62 cases of cardiorenal syndrome patients as the observation group A,88 cases of non cardiorenal syndrome patients as the observation group B,and the other 80 cases of healthy subjects the control group,and compare the changes of the indicators in each group. Results The observation group BNP precursor,Cystatin C,urea and creatinine levels were higher than the control group(P＜0.05); observation group A BNP precursor,Cystatin C,urea and creatinine levels were higher than the control group,the observation group B(P＜0.05); observation group B precursor BNP and cystatin C levels higher than the control group(P＜0.05),there is no significant difference between urea and creatinine(P＞0.05).Conclusion Elderly patients with chronic heart failure were associated with varying degrees of renal dysfunction,BNP cystatin C and high sensitivity,which can be used as an early indicator for the diagnosis of heart failure,which can be assessed in patients with renal damage,early diagnosis and treatment of cardiorenal syndrome has important significance.
