183例突发性聋患者焦虑抑郁状况分析

摘    要:目的 分析突发性聋患者焦虑抑郁的发生情况及其影响因素,为临床辅助治疗提供参考依据。方法 使用汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)及耳鸣残疾量表(THI),对突发性聋患者进行评估,并以健康且听力正常志愿者作为对照组,探究突发性聋患者发生焦虑抑郁情况、产生原因,及焦虑抑郁状态对疗效的影响。结果 实验组焦虑抑郁得分分别为(7.71±4.92,8.86±7.05),与对照组(4.00±2.26,3.44±2.48)得分比较,实验组焦虑抑郁得分显著高于对照组(P<0.05)。实验组焦虑抑郁人数所占比例显著高于对照组(P<0.05)。突聋严重程度、耳鸣严重程度、突聋患者受教育程度及突聋治疗效果与焦虑抑郁发生率有关(P<0.05),眩晕、耳闷感与突聋患者焦虑抑郁率无关(P>0.05)。结论 突聋患者较正常人更易产生焦虑抑郁情绪,且耳聋与耳鸣程度越大、受教育程度越高、治疗效果越差,焦虑抑郁情绪越易发生,伴随症状眩晕、耳闷感不是突聋患者发生焦虑抑郁情绪的必然危险因素。

Abstract:Objective To analyze the occurrence and influencing factors of anxiety and depression in sudden deafness patients,and to provide reference for clinical adjuvant therapy.Methods Hamilton Anxiety Scale (HAMA),Hamilton Depression Scale (HAMD) and tinnitus disability scale (THI) were used to evaluate the patients with sudden deafness.Healthy and normal hearing volunteers were used as the control group to investigate whether sudden anxiety and depression in deaf patients,their causes and the effect of anxiety and depression on the curative effect.Results The scores of anxiety and depression in the experimental group were 7.71±4.92 and 8.86±7.05,respectively.Compared with the control group (4.00±2.26,3.44±2.48),the anxiety and depression scores in the experimental group were significantly higher than those in the control group (P<0.05).The proportion of anxiety and depression in the experimental group was significantly higher than that in the control group (P<0.05).The severity of sudden deafness,the severity of tinnitus,the degree of education of patients with sudden deafness and the therapeutic effect of sudden deafness were related to the incidence of anxiety and depression (P<0.05),but not related to the anxiety and depression rate of patients with sudden deafness (P<0.05) .Conclusion Sudden deaf patients are more prone to anxiety and depression than normal people.And the greater the degree of deafness and tinnitus,the higher the level of education,the worse the treatment effect,the more prone to anxiety and depression.With the symptoms of dizziness,stuffy ears is not sudden deafness in patients with anxiety and depression the inevitable risk factors.
