锁骨远端钩钢板固定对肩部功能的影响观察

目的 探讨应用锁骨远端钩钢板内固定治疗锁骨远端骨折及肩锁关节脱位术后对肩部功能的影响。方法 回顾性分析2008-10～2015-12使用钩钢板固定治疗锁骨远端骨折及肩锁关节脱位患者的临床资料。37例患者男24例,女13例;年龄21～65岁,平均46岁。术中应用锁骨远端钩钢板治疗。术后定期随访,按照Constant评分标准进行肩关节功能评分。结果 术后32例患者得到随访,5例失访。随访时间6～24月,平均13月,14例X线出现肩峰下骨溶解,20例出现肩部疼痛及外展受限,肩关节Constant评分为72～90分,平均(82.0±3.0)分。结论 应用钩钢板治疗锁骨远端骨折及肩锁关节脱位需严格手术指征,细化术中操作,加强术后保护,以减少并发症的发生。

The influence on shoulder joint function after internal fixation with clavicular hook plate

Objective To investigate the influence on shoulder joint function after internal fixation with clavicular hook plate for distal clavicle fractures and acromioclavicular dislocations.Methods The data of 37 cases with distal clavicle fracture and acromioclavicular dislocation from October 2008 to December 2015 were analyzed retrospectively.There were 24 males and 13 females with an average age 46 years (range,21～65 years).All the cases were treated with clavicular hook plate and followed up regularly after surgical procedure.The shoulder joint function was evaluated according to constant shoulder score system.Results 32 cases were followed up and 5 cases were lost.The follow-up time ranged from 6 months to 24 months.14 cases of X-ray showed subacromial osteolysis,20 cases of shoulder pain and outreach was limited,shoulder constant score of 72～90 points,an average of(82.0 ± 3.0) points.Conclusion The application of hook plate for the treatment of distal clavicular fracture and acromioclavicular joint dislocation need to be strictly surgical indications,thinning operation,strengthen postoperative protection to reduce the incidence of complications.
