CYP11B2、CYP17基因多态性与PCOS肝经郁火证的相关性研究

摘    要:目的 通过对PCOS患者血清中CYP17、CYP11B2的基因检测,分析PCOS患者中医证型与基因表达的差异,为建立具有中医特色和优势的中医治疗方法提供客观依据。方法 收集2016-08~2018-01在延安市中医医院妇科就诊的肝经郁火证的PCOS患者115例作为研究组,选择同期因输卵管因素导致不孕的患者98例作为对照组。对CYP11B2、CYP17基因多态性及血睾酮水平进行检测,观察两组患者指标间的差异。结果 观察组中CYP11B2基因各基因型患者的血睾酮水平明显高于对照组,差异有统计学意义(P<0.01);观察组CYPl7基因CC基因型与等位基因C频率分布均明显高于对照组,差异有统计学意义(P<0.05),观察组CYPl7基因CC基因型患者血睾酮水平高于TC基因型,且观察组TT、TC、CC基因型患者中血睾酮水平随着等位基因C频率增加呈逐渐增加趋势(P<0.05)。结论 CYP11B2基因作为单一基因与肝经郁火证PCOS没有直接关联性;CYPl7基因与肝经郁火证PCOS有一定的关系,可能是肝经郁火证PCOS易感基因之一。

Abstract:Objective To analyzed the relationship of CYP11B2/CYP17 gene polymorphism and depressed Liver-dispersing and liver fire-purging therapy in patients with PCOS by detecting the levels of serum CYP17, CYP11B2 gene in patients for providing an Objective basis for establishment characteristics and advantages of Chinese medicine. Methods 115 PCOS cases with depressed Liver-dispersing and liver fire-purging therapy from August 2016 to January 2018 in Yan’an hospital of traditional Chinese medicine of gynecology clinic were selected as study group and 98 patients with tubal factor infertility at the same period as control group. The polymorphism CYP11B2, CYP17 gene and levels of serum testosterone were detected, and the differences between the two groups were observed. Results The serum testosterone level of CYP11B2 genotypes in the observation group were significantly higher than that in the control group, there difference was statistically significant (P<0.01). The frequency distribution of CYPl7 CC genotype and allele C in the observation group were significantly higher than those in the control group, the differences were statistically significant (P<0.05). The blood testosterone level of CYPl7 gene CC genotype was higher than that of TC genotype in the observation group, and the blood testosterone level of the TT, TC and CC genotypes in the observation group increased gradually with the increase of allele C frequency (P<0.05). Conclusion There is no direct relationship between CYP11B2 gene as a single gene and depressed liver-dispersing and liver fire-purging therapy in patients with PCOS. CYPl7 gene is certainly relating with depressed Liver-dispersing and liver fire-purging therapy in patients with PCOS and it may be one of susceptibility genes of the depressed Liver-dispersing and liver fire-purging therapy in patients with PCOS.
