单操作孔胸腔镜胸膜纤维板切除术治疗慢性包裹性胸腔积液25例

摘    要:目的 探索单操作孔胸腔镜胸膜纤维板切除术治疗慢性包裹性胸腔积液的可行性及手术方法。方法 选取2010年至今我科行全胸腔镜下胸膜纤维板切除术治疗的慢性包裹性胸腔积液患者41例,根据采取术式不同随机分为单操作孔组(25例)和三孔法组(16例)。回顾性分析对比两种术式的手术时间、术中出血量、术后拔管时间等指标。结果 单操作孔胸腔镜组较三孔法组术中出血量、平均手术时间、术后拔管时间等指标无显著统计学差异(P>0.05)。结论 单操作孔胸腔镜胸膜纤维板切除术治疗慢性包裹性胸腔积液可获得与三孔法相同的疗效,可以替代常规三孔法胸腔镜胸膜纤维板切除术治疗慢性包裹性胸腔积液。

Abstract:Objective To explore the feasibility and surgical method of thoracoscope fiberboard surgical treatment of chronic encapsulated pleural effusion with one-trocar hole.Methods Thirty-one patients who underwent cholecystectomy for pleural fibrosis in our department from 2010 to present were randomly divided into single-operation hole group (25 cases) and three-hole group 16 cases).Two kinds of operation time,intraoperative blood loss,extubation time and other indicators were retrospectively analyzed and compared.Results There is no significant difference between one-trocar hole group and three holes group in intraoperative bleeding,the average operation time,extubation time and other indicators(P>0.05).Conclusion The one-trocar hole has the same curative effect as the three holes method,and the method of the one-trocar hole can replace the conventional three holes method to cure the chronic encapsulated pleural effusion.
