45例窦口鼻道复合体骨折CT诊断分析

摘    要:目的:分析窦口鼻道复合体骨折的CT表现特征,以提高诊断水平。材料和方法:对371例颌面部外伤病人行螺旋CT横断位扫描,扫描后行冠状位重建(MPR)处理,对冠状位重建图像进行诊断和分析。结果:检出45例病人有窦口鼻道复合体骨折。其中单纯型骨折11例(24.44%),复合型骨折34例(75.56%),错位性骨折14例(31.11%),粉碎性骨折31例(68.89%);10例(22.22%)为双侧窦口鼻道复合体骨折,35例(77.78%)为单侧骨折。45例病人均合并颌面部其他部位骨折。所有病例均可在螺旋CT冠状位重建(MPR)图像中显示。结论:窦口鼻道复合体骨折以单侧复合型骨折最多见,骨折类型多为粉碎性。螺旋CT冠状位重建(MPR)图像能清楚显示窦口鼻道复合体骨折及窦口鼻道的阻塞情况,对于指导手术治疗及评价预后具有重要的临床价值。

Abstract:Purpose: The aim of this study was to present CT features of ostiomeatal complex fractures. Materials and Methods:371 subjects with maxillofacial trauma were undergone spiral CT.All the cases were diagnosed and analyzed on the coronal reconstruction(MPR) images.Results: 45 out of 371 cases were shown as ostiomeatal complex fractures, including 11(24.44%)simple fractures, 34(75.56%)complex fractures. 45 cases were composed of 14(31.11%)displaced fractures and 31(68.89%)comminuted fractures. 10 cases(22.22%)were identified as bilateral ostiomeatal complex fractures, and 35 cases(77.78%)as unilateral. All of cases with ostiomeatal complex fractures were accompanied with other maxillofacial bones fractures. All ostiomeatal complex fractures can be shown on coronal images of MPR by spiral CT.Conclusion: The most frequent, ostiomeatal complex fractures are characterized by unilateral complex comminuted fracture. Coronal CT (MPR) images can clearly show the ostiomeatal complex fractures as well as ostiomeatal obstruction, which is useful for the operations and the evaluation of prognosis.
