T淋巴细胞亚群变化与脓毒症发病的关系

目的探讨脓毒症发病过程中T淋巴细胞亚群变化及其临床意义。方法选取医院2010年1月-2013年1月间收治的脓毒症患者62例设为观察组，设同期非脓毒症的危重症患者60例为对照组，采用流式细胞仪检测2组患者24h外周血CD3＋（CD3阳性淋巴细胞）、CD4＋（CD4阳性淋巴细胞）、CD8＋（CD8阳性淋巴细胞）的百分比，及CD4＋／CD8＋值改变情况，研究数据采用SPSS12．0统计软件处理，以评价机体细胞免疫功能变化情况。结果观察组外周血CD3＋、CD4＋及CD4＋／CD8＋值均低于对照组，组间比较差异有统计学意义（P〈0．05）；观察组中死亡患者外周血CD4＋、CD8＋及CD4＋／CD8＋值均低于存活组，组间比较差异有统计学意义（P〈0．05）。结论脓毒症患者处周血CD3＋、CD4＋及CD4’／CD8＋值均显著降低，且死亡组患者上述指标显著低于存活组，表明脓毒症患者存在免疫功能损害，且外周血CD4＋、CD8＋及CD4＋／CD8＋值愈低，提示患者预后不佳。

Relationship between T lymphocyte subsets changes and sepsis pathogenesis

Objective To investigate the significance of T lymphocyte subgroup changes on the pathogenesis of sepsis in clinical. Methods A total of 62 cases of patients with sepsis admitted in our hospital from Jan,2010 to Jan,2013 were selected as observation group ,60 cases with sepsis critically in the same period were set as control group. The organism cellular immunity function was evaluated by detecting two groups of patients with 24 h peripheral blood CD3 ＋ （ CD3 posi- tive lymphoeytes ）, CD4 ＋ , CIM positive lymphocytes ）, the percentage of CD8 ＋ positive （ CD8 lymphocytes ）, and CD4 ＋ / CD8 ＋ value change situation. Results CD3 ＋ , CD4 ＋ and CD4 ＋/CD8 ＋ values of peripheral blood in the observation group were lower than those of the control group,with statistically significant differences between groups （P 〈 0.05 ） ; pe- ripheral blood CD4 ＋ , CD8 ＋ and CIM ＋/CD8 ＋values in death group of in patients were lower than those of the survival group, with statistically significant differences between groups （ P 〈 0.05 ）. Conclusion The fact that CD3 ＋ , CIM ＋ and CD4 ＋/CD8 ＋ were significantly lower in patients with sepsis, and death index was significantly lower than those patients in survival group, showed that patients with sepsis combined with immune function damage, and low peripheral blood CD4 ＋ , CD8 ＋ and CD4 ＋/CD8 ＋value,suggested with poor prognosis.
