QT 离散度预测老年急性心肌梗死患者发生室性心律失常的临床价值

摘    要:目的研究QT离散度预测老年急性心肌梗死患者发生室性心律失常的临床价值。方法选择140例老年急性心肌梗死患者作为研究对象,根据心电图检查结果分为校正的QT离散度(QTcd)≥60ms的观察组和QTcd〈60ms的对照组,观察住院期间室性心律失常发生情况及心功能指标。结果观察组患者发生室性期前收缩、非持续性室性心动过速、持续性室性心动过速、心室颤动的例数多于对照组(P〈0.05);左心室舒张末期内径(Left ventficular end diastolic diameter,LVEDD)、舒张晚期充盈峰速度(A峰)值均高于对照组(P〈0.05);舒张早期充盈峰速度(E峰)、E/A值、左室射血分数(Left Ventricular Ejection Fractions,LVEF)值均低于对照组(P〈0.05);QTcd与室性心律失常的发生呈正相关。结论QT离散度能够较好的预测老年急性心肌梗死患者室性心律失常的发生及心功能的改变,具有一定的临床价值。

Clinical analysis of QT dispersion in predicting ventricular arrhythmia in patients with acute myocardial infarction and its application value

Abstract:Objective To study the clinical value of QT dispersion in predicting ventricular arrhythmia in older patients with acute myocardial infarction and its application value. Methods Older patients with myocardial infarction were collected as the research object and divided into observation group( QTcd ≥60 ms) and control group (QTcd 〈 60 ms) according to the results of ECG examination. Then cases of ventricular arrhythmia and heart function index were observed. Results Ventricular premature beat, ventricular heartbeat tachycardia, ventrieular fibrillation cases of observation group were significantly more than those of control group ; LVEDD and A peak of observation group were significantly more than those of control group; E peak, E/A, LVEF were significantly lower than those of control group ; QTcd and ventricular arrhythmia had positive correlation. Conclusion QT dispersion had positive clinical value for it can predict ventricular arrhythmia and cardiac function.
