医护联合术前教育班对全膝关节置换患者术后康复的影响

目的探讨医护联合术前教育班对人工全膝关节置换术（TKA）患者术后康复的影响。方法选择不同时期行全膝关节置换术患者120例，将其随机分为对照组和干预组各60例。对照组给予常规术前指导．干预组病人术前进入医护联合术前教育班学习，内容包括有心理干预、身体评估、健康教育、功能训练。比较两组患者术前和术后2周（术后）的美国膝关节学会评分（AKSS）、膝关节活动范围（1KOM）、视觉模拟评分（VAS）和术后住院天数。结果与对照组相比，干预组术后AKSS和ROM明显增加，而VAS和住院天数明显降低，差异均有统计学意义，均P〈0．01（t=4．95，4．54，8．26，4．50）。结论医护联合术前教育班在工全膝关节置换术患者中的实施，可提高TKA术后患者的功能恢复效果．促进患者康复。

Effect of Preoperative Direction Cooperated by Doctors and Nurses an Postoperative Rehabilitation in Patients Receiving Total Knee Arthroplasty

Objective To investigate the effect of preoperative direction cooperated by doctors and nurses on postoperative rehabilitation in patients receiving total knee arthroplasty （TKA）. Methods A total of 120 cases scheduled for TKA were randomly assigned to the control and interventional group,each consisting of 60 cases. The control group received routine treatment and nursing care in orthopedics, while the interventional group received preoperative direction cooperated by doctors and nurses including health education, mental nursing and comprehensive fimctional exercise in addition. A comparison was conducted in both groups in terms of American Knee Society score （AKSS）, range of motion （ROM） and visual analogue scale （VAS） as well as postoperative length of stay before and two weeks after operation. Results The AKSS and ROM were considerably elevated in the interventional group while the VAS and postoperative hospital day decreased markedly when compared with those of the control group,showing significant differences between two groups, all P〈 0.01 （t=4.95, 4.54, 8.26, 4.50 respectively）. Conclusion The preoperative direction cooperated by doctors and nurses proved to increase the efficacy of the postoperative functional recovery in TKA patients, facilitating the early recovery of the patients.
