酒精性脂肪肝与肝硬化的超声诊断

本文报道经病理诊断的酒精性脂肪肝（ＡＦ）１０例，酒精性肝硬化（ＡＣ）１５例Ｂ超检查结果显示ＡＦ肝肿大，并有近场密集，远场衰减。ＡＣ示典型肝硬化表现。提出下列４点，有助于诊断ＡＦ与ＡＣ。①有明确嗜酒者，酒量＞１００ｇ／ｄ，５年以上；②ＡＦ者ＧＧＴ明显升高，ＡＣ轻度增高或正常；③Ｂ超结合病史有助于诊断ＡＦ及ＡＣ；④Ｂ超并可用于随访，ＡＦ戒酒后可完全恢复或明显好转。根据这些表现，作者认为Ｂ超是诊断ＡＦ与ＡＣ有价值的方法。

Evauation of Ultrasonography in Diagnosis of Alcoholic Fatty Liver and Liver Cirrhosis

Ten patients with alcoholic fatty liver(AF)and 15 alcoholic liver cirrhosis(AC) diagnosed clinically and histologically and studied by ultrasonography were analysed. The patients age ranged from 16-67 year(M:43 year). The amount of alcohol consumed by the patients AF and AC was not signficantly different. However, the duration of alcohol consumption was longer in AC than AF.On US scans,AF showed large enlargement of the Liver,with near field acoustic sonud condensed and far field markedly decresed. AC revealed a typical cirrhotic characteristics. The GGT levels were highly increased in AF,but slightly or not increased in AC patients. Based on these findings,the authors suggest that the following 4 points might be helful for the diagnosis of AF and AC:1. with heavy drinking >100g/d of alcohol over more than 5 years;2. highly incresing of GGT levels favorable for the diagnosis of AF;3. US scan is a favorable diagnostic tool for the diagnosis of AF and AC; 4. US may also be used for the follow-up study, after alcohol withdrawal, the AF may be completely recovered or markely improved. The authors considered that combined with the history of drinking and study of the GGT levels,US scans might be helpful for the diagnosis of AF and AC.
