螺旋CT诊断胆脂瘤型中耳炎的价值

目的:探讨螺旋CT诊断胆脂瘤型中耳炎的价值。材料和方法:回顾性分析经手术及病理证实的104例胆脂瘤型中耳炎的CT表现,探讨其诊断价值。结果:术前CT诊断骨壁受侵101耳,而术中发现为90耳,假阳性11耳,符合率为89．8％;术前CT显示胆脂瘤在中耳系统充填部位108耳,与手术结果对比,假阳性、假阴性各3耳,总体符合率达94．4％;术前CT诊断听骨链受侵234耳,而术中证实为215耳,假阳性22耳,假阴性3耳,符合率为89．3％。结论:螺旋CT诊断胆脂瘤型中耳炎,尤其对判定骨质破坏的范围和胆脂瘤的部位,制定手术方案和评估治疗效果,具有重要的指导意义。

The Value of Spiral CT in Diagnosing the Otitis Media with Cholesteatoma

Purpose: To explore the value of spiral CT in diagnosing the otilis media with cholesteatoma. Materials and Methods: Clinical and spiral CT manifestation of 104 cases of the otitis media with cholesteatoma were retrospectively analyzed. All the cases were confirmed by operation and pathology. Results:Confirmed by operation,for the bony-erosion the rate of conformation between the radiographic and operative findings was 89.8% ,94.4% for the position of soft-density focus,89.3% for the ossicles-erosion. Conclusion: Spiral CT has a role in diagnosing the otitis media with cholesteatoma and especially about the bony-erosion and the position of cholesteatoma.
