三维适形放疗联合康莱特注射液治疗腹膜后转移瘤的临床研究

目的：观察三维适形放疗联合康莱特（KLT）注射液治疗腹膜后转移瘤的疗效及不良反应。方法：52例腹膜后转移瘤患者，随机分为实验组和对照组，对照组给予6MV—X线局部三维适形放疗DT5000eGy／25次；实验组在局部放疗同时，每日静脉滴注康莱特注射液100ml，连用21天，观察临床疗效和不良反应。结果：共有50例患者完成实验，实验组和对照组患者腰背部疼痛缓解有效率分别为78．6％和59．1％，两组间差异无统计学显著性（P〉0．05）。肿瘤治疗的有效率实验组达82．1％，与对照组54．5％相比，有显著性差异（P〈0．05）。两组恶心呕吐的发生率皆比较高，但对照组Ⅲ、Ⅳ度恶心呕吐的发生率达36．4％，而实验组仅为7．1％，两组间有显著性差异（P〈0．05）。两组间白细胞减少有显著性差异（P〈0．05），实验组白细胞下降的发生率及下降程度皆较对照组轻。结论：KLT联合局部放疗较单纯放疗在控制肿瘤的近期疗效及缓解患者疼痛方面表现出协同治疗的优势，可降低恶心呕吐的反应程度，保护骨髓的造血功能。

Clinical study of three- dimensional conformal radiotherapy combined with Kanglaite in the treatment of retroperitoneal metastatic tumor

Objective ：To investigate the efficacy and adverse reaction of three - dimensional conformal radiothera- py（3D - CRT） combined with Kanglaite （KLT） in the treatment of retroperitoneal metastatic tumor, Methods ： Fifty - two patients with retroperitoneal metastatic tumor were divided into two groups. The control group was given 3D - CRT DT 5000cGy/25 fractions,while the sthdy group was given KLT injection in addition to 3D - CRT. The clinical outcome and adverse reaction were observed. Results： Fifty patients had completed the study. The response rates of pain relief for study group and control group were 78.6% and 59.1% respectively （P 〉 0.05 ）. The response rates of tumor were 82.1% and 54.5% （P 〈0.05）. The incidence of nausea and vomiting for all the cases was high,but the degree of llland 1V in the study group was significantly lower than those in the control group（P 〈0.05）. The inci- dence of leukopenia in the study group was also significantly lower than those in the control group. Conclusion ： KLT combined with 3D - CRT can improve the clinical efficacy of retroperitoneal metastatic tumor. The adverse reactions are lower and the tolerance is well.
