摘    要:目的:观察肠饲谷氨酰胺(GLN)对烧伤大鼠肠粘膜的保护作用。方法:采用30%Ⅲ°烧伤大鼠早期肠道喂养模型,将20只大鼠随机分为GLN组(n =10)和NON-GLN(n =10)组,一周后处死动物,无菌取肠系膜淋巴结行细菌培养,统计细菌移位率;取门静脉血检测内毒素;对回肠粘膜行光镜检查,并用显微图像分析仪对大鼠回肠粘膜的厚度、绒毛高度和隐窝深度进行计算机图像分析。结果:GLN组大鼠回肠粘膜的厚度、绒毛高度和隐窝深度较NON-GLN组明显增加;GLN组门静脉血内毒素含量及GLN组肠系膜淋巴结细菌移位率明显低于NON-GLN组。结论:提示肠道补充GLN能有效地维护严重烧伤后肠道粘膜的结构,减少细菌移位和减少内毒素进入门静脉。

Abstract:Objective: To investigate the protecting effect of L-glutamine (GLN) on intestinal mucosal damage in severe burn of rats. Methods: 20 Wistar rats subjected to 30% TBSA full thickness burns were randomly divided into GLN group (n =10) and NON-GLN group ( n =10). Forage enriched with 1.5%GLN was fed to GLN group and equal amount of glutamine free forage was given to NON-GLN group. Seven days later, all rats were killed. Under aseptic condition, the mesenteric lymph nodes were harvested, weighed, homogenized and cultured. Bacterial translocation rate was evaluated. Portal vein plasma endotoxin levels were determined with chromogenic limulus amebocyte lysate (LAL) technique. Terminal ileum specimens were sampled for histological examination and microscopical image scanning analysis. Results: Thickness of mucosa, height of villus and depth of crypt decreased more obviously in NON-GLN group than those in GLN group. The levels of plasma endotoxin and the bacterial translocation incidence were decreased obviously in GLN group in comparison with those in NON-GLN group. Conclusion: It is suggested that oral feeding of GLN can maintain the mucosa integrity effectively in severely burned rats and effectively decrease gut-originated bacterial/endotoxin translocation.
