摘    要:对33例复杂部分发作癫痫患者进行单光子计算机断层摄影(SPECT)、磁共振(MRI)和脑电频谱分析(PSA)检查。其中有30例在发作间期进行SPECT检查,均显示颞叶局灶性脑血流减低,14例定位在左侧,10例在右侧,6例为双侧病灶。MRI结果显示26例存在颞叶异常改变(16例在左侧,7例在右侧,3例为双侧病灶)。PSA发现30例有明确的病灶(11例在左侧,16例在右侧,3例为双侧)。SPECT和PSA结果完全或部分一致的占73.4%,MRI和SPECT完全或部分一致的占76.9%,MRI和PSA完全或部分一致的占55.1%,34.6%的病人3种检查定位完全一致。

Abstract:The focal localisation of 33 patients with complex partial seizures(CPS)was determined with EEG power spectral analysis (PSA). magnetic resonance imaging (MRI) and single photon emission computerised tomography (SPECT). Thirty patients had epileptiform discharges in EEG examinations (11 patients in left side, 16 in right side,and 3 in both sides). Twenty-six receiving MRI scanning had abnormal results(16 patients in left temporal lobes,7 in right temporal lobes,and 3 in bilateral temporal lobes). All of the 30 patients showed reduced regional cerebral blood flow(rCBF) in SPECT examination (14 patients in left side, 10 in right side,and 6 in both sides). The complete and partial concordant rate was 73.4%(PSA and SPECT) ,76. 9% (MRI and SPECT)and 55.1%(PSA and MRI),respectively. The three methods were congruent completely in 34. 6 % patients. It was suggested that combination of PSA,MRI and SPECT is the most useful method in localising epileptogenic foci for surgical candidates.
