移植复苏后进一步卵裂胚胎可提高冻融胚胎移植临床妊娠率

摘    要:目的探讨复苏后胚胎进一步卵裂情况对冻融胚胎移植妊娠结局的影响。方法对2010年6月~2011年12月间我院生殖中心行的191例FET周期进行回顾性分析:于移植前一天下午解冻胚胎,过夜培养,观察卵裂球进一步卵裂情况,然后按每一例FET所移植胚胎的进一步最多卵裂个数(nmax,即该患者所移植胚胎中进一步卵裂胚胎数为该数目)分为A组(nmax=0,n=31)、B组(nmax=1,n=58),C组(nmax=2,n=69),D组(nmax=3,n=33),观察不同卵裂球进一步卵裂情况对临床妊娠结局的影响。结果各组临床妊娠率分别为:A组29.0%(9/31),B组35.6%(21/58),C组40.6%(28/69),D组45.5%(15/33),两两比较差异有统计学意义(p<0.05)。结论冻融胚胎移植提前解冻培养、选择进一步卵裂的胚胎进行移植有助于提高FET周期临床妊娠率。

Abstract:Objective To evaluate the effects of cryopreserved embryos with further cleavage after thawing on clinical outcomes undergoing frozen embryo transfer cycles. MethodsA retrospective study was programmed in our center,including 191 FET cycles from June 2010 to October 2011.Measurements were analyzed according to the maximal further blastomere counts after thawing of each patient: group A(nmax=0,n=31)、group B(nmax=1,n=58)、group C(nmax=2,n=69) and group D(nmax=3,n=33).Each patient was analyzed for oocyte yield,2PN rate,cleavage 2PN embryos,high quality embryos,clinical pregnancy rate. ResultsSignificant difference was observed among groups 29.0%(9/31),35.6%(21/58),40.6%(28/69) and 45.5%(15/33) in group A,B,C and D,respectively,p <0.05]. ConclusionCryopreserved embryos with further cleavage after thawing increases clinical outcome of FET cycles.
