经阴道彩色多普勒超声在早期输卵管妊娠诊断中的价值

目的探讨经阴道彩色多普勒超声(TVCDS)对早期输卵管妊娠的诊断价值,旨在提高早期异位妊娠的诊断水平。方法回顾性分析89例我院行TVCDS检查并经临床或手术病理所证实的早期输卵管妊娠的声像图特征。结果经TVCDS检查提示早期输卵管妊娠86例,误诊2例,漏诊1例,诊断符合率96.6%;声像图表现为宫内无孕囊,附件区包块,子宫内膜增厚,盆腔少量积液,附件区包块内和/或周边可见彩色血流显示,脉冲多普勒(PW)检测出低阻力的动脉血流频谱。结论 TVCDS检查简单、及时,对于盆腔包块、盆腔积液的检出极为敏感,再辅以彩色多普勒血流显像(CDFI),对早期输卵管妊娠的诊断具有重要价值,应列为首选的可靠检查方法。

Transvaginal Color Doppler Sonography in Diagnosis of Early Tubal Pregnancy

Objective To evaluate the clinical value of transvaginal color Doppler sonography(TVCDS)in diagnosing early tubal pregnancy to improve the diagnosis of early eccyesis.Methods Retrospectively analyzing 89 cases of early tubal pregnancy detected by preoperative transvaginal color ultrasonography and confirmed by operation and pathology.Results 86 cases were diagnosed as early tubal pregnancy by transvaginal color Doppler sonography,2 cases were misdiagnosed,and one case was missed.The coincidence rate of diagnosis was 96.6%.Ultrasonography displayed that no intrauterine pregnancy sac,pelvic mass,endometrial thickening,a small amount of pelvic effusion.Pelvic mass peripheral and internal had blood supply.PW detected low resistance arterial flow spectrum.Conclusion TVCDS is simple,timely,and extremely sensitive to pelvic mass and pelvic effusion.Supplemented by color Doppler flow imaging(CDFI),TVCDS is significantly valuable as a preferred and reliable method in early diagnosis of tubal pregnancy.
