摘    要:目的研究不同浓度尿激酶配置液封管法对中心静脉导管血栓形成所致堵管后溶栓再通的效果。方法本研究为单中心、开放、随机对照研究。将158例中心静脉导管血栓性堵管患者随机分为A、B、C三组,其中A组52例,B组54例,C组52例,A组采用含25000U/ml尿激酶配置液封管溶栓,B组采用含10000U/ml尿激酶配置液封管溶栓,C组采用5000U/ml尿激酶配置液封管溶栓,比较三组溶栓再通率、溶栓后凝血功能。结果 A组溶栓再通率高于B组及C组,B组溶栓再通率高于C组(P<0.05),且溶栓后2h凝血酶原时间(PT),活化部分凝血酶原时间(APTT)的改变无统计学意义(P>0.05)。结论含25000U/ml尿激酶封管液进行中心静脉导管溶栓,再通效果好,费时短,且不影响凝血功能。

Thrombolytic Effect of Urokinase at Different Concentration in Tube Sealing on Central Venous Catheter Thrombosis in Hemodialysis

Abstract:Objective To explore the thrombolytic effect of urokinase at different concentration in tube sealing on central venous catheter thrombosis in hemodialysis. Methods 158 cases with central venous catheter thrombosis were randomly divided into 3 groups: Group A (n=52, tube sealing liquid with urokinase of 25000U/ml), Group B (n=54, tube sealing liquid with urokinase of 10000U/ml) and Group C (n=52, tube sealing liquid with urokinase of 5000U/ml), and the rates of thrombolysis and reeanalization of the 3 groups were compared as well as the blood coag- ulations. Results The rate of thrombolysis and recanalization of Group A was higher than those of the other 2 groups while the rate of Group B was higher than that of Group C (P〈0.05); the changes in prothrombin time (PT) 2 hours after thrombolysis and in activated partial thromboplastin time (APTT) were of no statistical significance (P〈0.05). Conclusions The tube sealing liquid with urokinase of 25000U/ml is of good effect on recanalization of central venous catheter thrombosis, and it can save the time for patients and will not affect the blood coagulation.
