CO2激光与保守药物方案治疗血管性声带炎的临床疗效比较

目的 探讨CO2激光与保守药物方案治疗血管性声带炎的临床疗效.方法 选取2016年6月至2017年6月乐山市人民医院耳鼻咽喉头颈外科收治的血管性声带炎患者60例为研究对象,采用随机数字表法分为对照组和试验组,每组各30例.对照组实施保守药物治疗,试验组在对照组治疗基础上给予CO2激光照射治疗,观察治疗后两组患者声门大小评估结果、嗓音听感知评估(RB H)结果、不良事件发生率、住院时间及治疗后1个月随访情况.结果 治疗前两组患者发声时声门闭合程度评分、RB H评估结果比较,差异无统计学意义(P>0.05);治疗后两组患者发声时声门闭合程度评分较治疗前明显降低,RB H评分较治疗前明显降低,而治疗后试验组R、B、H评分较对照组明显降低,差异有统计学意义(P<0.05);治疗后两组患者发声时声门闭合程度评分比较,差异无统计学意义(P>0.05);治疗后试验组不良事件发生率(16.67%)、复发率(10.00%)较对照组不良事件发生率(43.33%)、复发率(33.33%)明显降低,差异有统计学意义(P<0.05);两组患者住院时间比较,差异无统计学意义(P>0.05).结论 CO2激光治疗血管性声带炎在改善患者声门大小及嗓音听感知方面的临床疗效较保守药物更明显,可有效降低不良事件发生率及复发率,临床积极作用明显.

A Comparative Study of the Clinical Efficacy of CO2 Laser and Conservative Drug Regimen in the Treatment of Vascular Chorditis

Objective To investigate the clinical efficacy of CO2 laser and conservative drug regimen in the treatmentofvascularchorditis.Methods 60caseswithvascularchorditistreatedinLeshanPeople'sHospitalfrom June of 2016 to June of 2017 were selected as the subjects and divided randomly into the control group and the experiment group by the random number table method .The control group was treated with conservative drugs while the experiment group was treated with CO2 laser irradiation based on the treatment of conservative drugs .The size of the glottis ,evaluation of Roughness ,Breathiness ,Hoarseness (RBH) System ,the incidence of adverse events , 1-month follow-up results and length of hospital stay in the two groups were observed after treatment .Results There were no significant differences between the two groups in the scores of glottal closure while producing sound and evaluation results of RBH before treatment (P> 0.05) .After treatment ,the scores of glottal closure while producing sound were decreased significantly in the two groups (P<0.05) and the evaluation results of RBH were increased significantly (P<0.05) .The scores of roughness ,breathiness ,hoarseness were significantly higher in the experiment group than in the control group after treatment (P<0.05) .There was no significant differences in the degree of glottal closure between the two groups after treatment (P>0.05) .The total incidence of adverse events (16.67% ) and recurrence rate (10.00% ) in the experiment group were significantly lower than those in the control group which were 43.33% and 33.33% respectively (P<0.05% ) .There was no significant differences between the two groups in the length of hospital stay (P>0.05) .Conclusion The CO2 laser regimen is more effective than the conservative drug regimen in the treatment of vascular chorditis by effectively reducing the incidence of adverse events and recurrence rate .
