孕早期血中孕酮水平对妊娠结局的影响

目的探讨孕早期血中孕酮水平在维持妊娠中的作用及对妊娠结局的影响。方法将在我院就诊的孕早期患者中血孕酮水平低于63.6 nmol/L、孕8周仍无原始心管搏动、或者尿妊娠试验阳性而B超未发现子宫腔内有妊娠囊的140例患者,随机分为两组(各70例)。治疗组用黄体酮注射液+绒促性素治疗;对照组不用黄体酮注射液,只用绒促性素进行治疗。均将10天作为1个疗程,第1个疗程结束后复查血中孕酮水平,同时用B超监测胚胎发育情况。结果治疗组中68例继续妊娠至分娩,2例胚胎未发育;对照组中41例继续妊娠,28例发展为难免流产,1例输卵管妊娠。结论孕早期,血中孕酮水平低于63.6 nmol/L者发生流产的机会较多;黄体酮注射液+绒促性素的治疗可以改善妊娠结局,降低流产的发生。

The effect of progesterone in blood of early trimester of pregnancy on the pregnancy outcome

Objective To investigate the effect of progesterone in blood of early trimester of pregnancy on thepregnancy maintenance and thepregnancy outcome.Methods 140 patients whose progesterone below 63.6 nmol/L,no heart tube impulse within 8 weeks or pregnancy testof urine is positive butthe blastula were not detected by B ultrasoundwere divided into two groups,respectively70 cases.The treatment group was treated with progesterone and chorionic gonadotropic hormone,the control group was treated with chorionic gonadotropic hormone but without progesterone.10 days was a course of treatment,rechecked the progesterone in blood meanwhile detected the embryo by B ultrasound after the first course of treatment.Results 68 cases in the treatment group continued pregnancy to deliver,2 patients' embryo didn't develop.41 cases in the controlgroupcontinued pregnancy,28 cases developed to inevitable abortion,and1 case was oviductal pregnancy.Conclusion The patients whose progesterone in blood of early trimester of pregnancy below 63.6 nmol/L have more chances of abortion,the treatment with progesterone and chorionic gonadotropic hormonecan improve the pregnancy outcome and decrease the occurrenceof abortion.
