鼻内窥镜在鼻出血诊治中的临床应用

目的研究鼻内窥镜在鼻出血诊断与治疗中的应用,并与前鼻镜比较。方法选择在2004年5月~2006年1月间263例非外伤性鼻出血患者,随机分为对照组与实验组,分别采用前鼻镜检查与鼻内镜检查处理,了解鼻出血的主要病因,比较两组诊断符合情况、首次确诊情况以及采用鼻腔填塞或后鼻孔填塞的情况。结果病因多数为Little区黏膜病变,此外有鼻腔鼻窦炎症、鼻中隔偏曲、良性与恶性肿瘤等。试验组在首次确诊率、鼻腔填塞、后鼻孔填塞率等指标上,与对照组比较差异均具有统计学意义（P〈0.01）。结论与传统的前鼻镜相比,鼻内镜在鼻出血的临床诊治中具有确诊早、创伤小、效果好的优点,是首选方法。

Nasal endoscopy for diagnosis and therapy of epistaxis

Objective To study the clinical applications of the nasal endoscopy for the diagnosis and therapy of epistaxis.Methods Two hundreds and sixty-three cases of nontraumatic epistaxis during May 2004 to Jan.2006 were randomly divided into the two groups.The control group received the anterior rhinosopy and the trial group was performed the nasal endoscopy.To know the common causes of epistaxis and to compare the diagnostic correspondence,final diagnosis and the effects of nasal packing and postnarial packing.Results The main cause of epistaxis was the mucosal diseases on Little area.In addition,septal deviation,inflammation of nose and sinuses,benign and malignant neoplasia were found.The differences in diagnostic comfirmation for the first nasal endoscopy,nasal or postnarial packings between the two groups had statistical significance(P<0.01).Conclusion Nasal endoscopy has the advantages of earlier diagnosis,less injury and better outcome.It should be recommended for the diagnosis and treatment of nasal bleeding.
