视黄醇结合蛋白、前白蛋白对胎儿宫内营养状况评估价值研究

摘    要:目的 探讨视黄醇结合蛋白(RBP)、前白蛋白(PA)评价胎儿宫内营养状况的临床价值.方法 选择2015年在淮北市妇幼保健院分娩的338例新生儿作为研究对象,按胎龄分为早产儿组137例和足月儿组201例,生后12h内取静脉血检测RBP、PA水平,比较不同分组新生儿RBP、PA水平并分析RBP、PA水平对胎儿宫内营养状况的评价效果.结果 足月儿组血清RBP和PA水平均显著高于早产儿组(t值分别为5.548、4.337,均P<0.05),35~36周早产儿组血清RBP、PA水平均显著高于28~34周早产儿组(t值分别为5.097、9.007,均P<0.05),低体重儿组新生儿血清RBP、PA水平均显著低于其他两组RBP:t值分别为5.379、5.882,均P<0.05;PA:t值分别为6.498、7.267,均P<0.05),而巨大儿组新生儿血清RBP、PA水平均显著高于其他两组(t值分别为4.276、5.034,均P<0.05).结论 血清RBP、PA综合评价能够准确快速判断胎儿宫内营养状况,在临床上具有重要应用价值.

Abstract:Objective To investigate the clinical value of retinol binding protein ( RBP ) and prealbumin ( PA ) in evaluating fetal nutritional status .Methods Totally 338 neonates born in Huaibei Maternal and Child Health Hospital in 2015 were divided into premature group (n=137) and term group (n=201) according to gestational age.The levels of RBP and PA in venous blood were measured within 12 hours after birth .The levels of RBP and PA in different groups were compared and the correlation between levels of RBP and PA and fetal intrauterine nutrition status was analyzed .Results Serum levels of RBP and PA in term group were significantly higher than those in premature group (24.14 ±5.67mg/mL vs.17.35 ±4.32mg/mL, 137.54 ±19.24mg/L vs.106.74 ±17.51mg/L, t value was 5.548 and 4.337, respectively, both P <0.05).The serum levels of RBP and PA in premature neonates with gestational 35 -36 weeks were significantly higher than those in neonates with gestational 28-34 weeks group (18.88 ±3.69mg/mL vs.15.27 ±5.18mg/mL, 114.79 ± 18.34mg/L vs.95.78 ±16.38mg/L, t value was 5.097 and 9.007, respectively, both P<0.05).Serum levels of RBP and PA in low birth weight group were significantly lower than those in other two groups (16.71 ±5.98mg/mL vs.24.95 ±4.01mg/mL, 26.82 ±4.28 mg/mL, t value was 5.379 and 5.882, respectively, both P<0.05;95.87 ±15.82mg/L vs.134.85 ±18.96mg/L, 150.27 ±19.54 mg/L, t value was 6.498 and 7.267, respectively, both P<0.05).The serum levels of RBP and PA in macrosomia group were significantly higher than those in other two groups (t value was 4.276 and 5.034, respectively, both P<0.05).Conclusion Comprehensive evaluation of serum retinol binding protein and prealbumin can accurately and quickly judge the nutritional status of fetus , which has important application value in clinical practice .
