黄芪桂枝五物汤加味治疗类风湿关节炎临床研究

摘    要:目的:观察黄芪桂枝五物汤加味治疗类风湿关节炎的临床疗效及其对患者血沉(erythrocyte sedimentation rate,ESR)和C反应蛋白(C reactive protein,CRP)水平的影响。方法:将76例类风湿关节炎患者按照随机数字表法分为对照组和治疗组,每组各38例。对照组应用氟米特片、甲氨蝶呤等常规西药治疗,治疗组在对照组的基础上给予黄芪桂枝五物汤加味治疗。检测两组患者治疗前后ESR、CRP水平,比较两组患者的临床疗效、疾病进展情况及临床症状积分变化情况。结果:两组患者治疗后CRP和ESR水平低于本组治疗前,且治疗组治疗后低于对照组治疗后,差异均有统计学意义(P<0.05)。对照组有效率为81.6%,治疗组有效率为92.1%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后缓解期及轻度活动期例数多于本组治疗前,治疗组缓解期例数多于对照组,差异具有统计学意义(P<0.05)。治疗组关节疼痛、晨僵、压痛、肿胀和功能障碍评分低于对照组,差异具有统计学意义(P<0.05)。结论:黄芪桂枝五物汤加味治疗类风湿关节炎,可以改善患者临床症状及CRP、ESR水平,减缓疾病进展。

Abstract:Objective:To observe the clinical curative effect of Flavored Astragalus and Cinnamon Twig Five Substances Decoction on rheumatoid arthritis(RA)and its influence on ESR and CRP of the patients.Methods:A total of 76 patients with RA were randomly divided into the control group and the treatment group,with 38 cases in each group.The control group was treated with routine Western medicine such as flunomide tablets and methotrexate,while the treatment group was treated with Flavored Astragalus and Cinnamon Twig Five Substances Decoction on the basis of the control group.The levels of erythrocyte sedimentation rate(ESR)and C reactive protein(CRP)of the two groups were detected before and after the treatment.The clinical efficacy,disease progression and clinical symptom score change of the two groups were compared.Results:The levels of CRP and ESR in the two groups after the treatment were lower than those of the same group before the treatment,and the above indexes in the treatment group were lower than those in the control group after treatment,and all the differences were statistically significant(P<0.05).The effective rate of the control group was 81.6%,and that of the treatment group was 92.1%.The difference was statistically significant(P<0.05).The number of patients in remission stage and mild active stage after the treatment in the two groups was more than that before the treatment in the same group,and the number of cases in remission stage in the treatment group was more than that in the control group,the difference was statistically significant(P<0.05).The scores of joint pain,morning stiffness,tenderness,swelling and dysfunction in the treatment group were lower than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:In terms of RA,Flavored Astragalus and Cinnamon Twig Five Substances Decoction can improve the clinical symptoms and CRP,ESR levels,and slow down the disease progression.
