腭裂修复术后并发症的临床分析

目的:分析腭裂术后并发症的原因,提高手术质量。方法:总结我院1993-10—2008-05共601例腭裂修复手术病例,从患者年龄、腭裂类型、部位等方面进行分析统计。结果:601例患者中,学龄前组为482例,发生穿孔40例,学龄后组119例,发生穿孔4例。病变大多发生在腭垂部,其次为软硬腭交界部;601例患者中,术后出血32例,其中以鼻腔侧出血多见,其次见于硬腭的松弛切口和侧切口。结论:腭裂术后穿孔或复裂与患者年龄及腭裂程度及手术技巧有关;术后出血与术中操作不当,止血不彻底有关。提高手术技巧,加强术后护理和作好出院指导可有效预防腭裂术后并发症的发生。

Clinical Analysis of Postoperative Complications in Cleft Palate Patients

Objective:To analyze the reasons of postoperative complications in palatoplasty and to improve the quality of reconstructive measures. Methods: A retrospective study of 601 cases from October 1993 to May 2008, 76 cases with postoperative complications were analysed. Results: In 482 cases of pre-school children, 40 cases occurred postoperative perforation; in 119 cases of school children, 4 cases occurred perforation. Most perforation site located in the uvula, the junction of hard and soft palate was next. In 32 cases of postoperative hemorrhage, nasal bleeding were the most common, followed by bleeding at the relaxation incision site and/or lateral incision site. Conclusion: The postoperative perforation may correlated with the age and the severity of clefts. The bleeding may correlated with improper operative measures. Improve the surgical technique, postoperative care and strengthen guidance during discharge may be effective in preventing the occurrence of postoperative complications
