奥沙利铂联合亚叶酸钙与5-氟脲嘧啶治疗老年晚期结直肠癌的临床研究

目的评价国产奥沙利铂(L-OHP)联合亚叶酸钙(CF)、5-氟脲嘧啶(5-Fu)治疗局部晚期或转移性老年结直肠癌患者(≥70岁)的临床疗效及毒副反应。方法入组30例患者,方案L-OHP 130mg/m2静脉滴注2h,第1天;CF 200mg/m2静脉滴注2h,第1~5天;5-Fu 425mg/m2静脉滴注6h,第1~5天。每3周重复,两周期后评价疗效。结果30例患者均可评价疗效,无CR病例,PR 15例,SD8例,PD 7例,有效率为50%,中位生存期18个月,中位无进展生存9.5个月,1年生存率为66.7%。主要毒副反应为神经毒性,中性粒细胞减少,消化道反应及腹泻。结论L-OHP联合5-Fu/FA方案治疗局部晚期或转移性老年结直肠癌患者疗效较高,耐受性较好,值得进一步研究。

The Clinical Study of 5-Fluorouracil,Calcium Folinate and Oxaliplatin in Old Patients with Advanced Colorectal Cancer

Objective To evaluate the efficacy and toxicity reaction of 5-Fluorouracil Calcium folinate and Oxaliplatin in old patients with advanced colorectal cancer.Methods Thirty patients over 70 years old with advanced colorectal cancer were enrolled.The chemotherapy regimen consisted of Oxaliplatin 130mg/m~2 iv.gtt.2 hours d1,CF 200mg/m~2 iv.gtt.2 hours d1-5,5-Fu 425mg/m~2 iv.gtt.6 hours d1-5.The patients were repeatedly treated every 3 weeks and evaluated the efficacy after 2 cycles.Results All eligible patients were assessed for response,none achieved a complete response,15 patients achieved a confirmed partial response.The overall response rate was 50%,the median PFS was 9.5 month,the median OS was 18 month and the 1 year survival rate was 66.7%.Conclusion 5-Fluorouracil,Calcium folinate and Oxaliplatine represents a well-tolerated active palliative regimen for patients with colorectal cancer over 70 years old.
