运用PDCA循环持续提高医务人员手卫生依从性

目的通过持续质量改进，提高全院医务人员的手卫生依从性，有效降低医院感染的发生率。方法应用计划-实施-检查-处理（PDCA）循环即管理循环模式进行持续质量改进，加强全员培训，注重过程管理，强调规范洗手，并定期检查和随机抽查。结果实施PDCA循环后，医务人员的手卫生知识掌握率由48.00％上升至63.99％；手卫生依从率由实施前的65.11％上升至实施后的82.40％，差异均有统计学意义（χ2值分别为12.75、259.65，均P＜0.05）。1 000住院床日速干手消毒剂消耗量递增明显，实施后是实施前的2.95倍。结论应用PDCA循环进行持续质量改进，可有效提高医务人员手卫生意识及其依从性。

Application of PDCA cycle to improve hand hygiene compliance among health care workers

Objective To improve hand hygiene compliance among health care workers（HCWs）through continuous quality improvement,and effectively reduce the incidence of healthcare-associated infection（HAI）.MethodsContinuous quality improvement was performed by adopting plan-do-check-action（PDCA）cycle,all HCWs were trained,hand hygiene was stressed,periodical and random checking was conducted.Results After the implementation of PDCA cycle,the acknowledge rate of hand hygiene enhanced from48.00%to 63.99%;hand hygiene compliance rate enhanced from65.11%to 82.40%,the difference were both significant（χ^2=12.75,259.65,respectively,both P〈0.05）.The daily consumption of instant hand antiseptic per 1 000 bed day increased obviously,which was 2.95-fold of pre-implementation.Conclusion Continuous quality improvement through PDCA cycle can effectively improve hand hygiene compliance rate of HCWs.
