高血压病左室肥厚患者心脏功能与中医证型的相关性研究

目的:探讨心脏功能对高血压病左室肥厚患者中医临床辨证分型的影响。方法:应用心脏彩色多普勒超声指标,对138例高血压病左室肥厚患者进行观察,分析其对中医辨证分型的影响。结果:肝火亢盛证是EH-LVH早期的主要证候特征,而阴阳两虚和痰湿壅盛则为较后期的主要证候特征。EH-LVH早期呈肝火亢盛实证表现时,对心脏收缩舒张功能影响较小,而阴阳两虚时多属高血压病后期,反映出心脏收缩舒张功能明显异常。结论:不同中医证型EH-LVH患者均有不同程度左室重构和舒张功能减退的超声心动图表现。心脏收缩舒张功能可以作为高血压病的中医证型的客观化指标。

Study on Correlation between Heart Function and TCM Types of Syndromes of Essential Hypertension with Left Ventricular Hypertrophy

Objective:To discuss the impact on heart function between TCM clinical syndrome type of hypertension patients with left ventricular hypertrophy.Methods: 138 cases of hypertension patients with left ventricular hypertrophy were examined by doppler echocardiography.Results: Live-fire hyperactivity type is the main characteristic syndrome in EH-LVH patients in the early stage,excessive accumulation of phlegm-dampness type and deficiency of both Yin and Yang type are the main syndromes in the later stage.EH-LVH patients with Live-fire hyperactivity type in early stage less affected the systolic and diastolic function.EH-LVH patients with both Yin and Yang type affected the systolic and diastolic function obviously.Conclusion:EH-LVH patients with different TCM syndromes have different left ventricular reconstitution and manifestations of decline of diastolic function in ultrasonic cardiogram.Systolic and diastolic heart function could be used as objective indicators on TCM types of syndromes of essential hypertension.
