aVR导联ST段抬高的急性前壁心肌梗死患者临床资料分析

目的探讨心电图aVR导联ST段抬高对急性前壁心肌梗死患者临床预后的价值。方法根据心电图aVR导联ST段变化，将78例急性前壁心肌梗死患者分为抬高组41例和无抬高组37例。分析两组患者的心电图、冠状动脉造影及心血管事件发生率。结果①左主干病变9例，ST段抬高组8例（19．51％）、ST段无抬高组1例（2．7％），两组统计有显著性差异（P〈0．05）；多支冠状动脉血管病变44例，ST段抬高组28例（68．29％）、ST段无抬高组16例（43．24％），两组统计有显著性差异（P〈0．05）；②心血管事件发生率12例，ST段抬高组8例（19．51％）、ST段无抬高组4例（10．81％），两组有显著性差异（P〈0．01）。结论aVR导联ST段抬高对预测急性前壁心肌梗死患者的预后有重要的价值。

Analysis of clinical data about ST segment elevation of aVR lead in patients with acute anterior wall myocardial infarction

Objective To explore the value of ST segment elevation of aVR lead in patients with acute anterior wall myocardial infarction. Methods The electrocardiogram,coronary angiographic findings and the rate of heart events were analyzed in 78 patients with first episode of anterior wall acute myocardial infarction , divided into elevation group and no drift group according to the ST segment changes of aVR lead. Results The patients whose infarct- related vessel was left main coronary artery were significantly more in elevation group (8 cases, 19.51%) than in no drift group (1 cases ,2.7%) (P<0.05) .The cases with multi- vessel lesions in elevation group (28cases , 68.29%) were significantly more than in no drift group (16cases , 43.24%) (P<0.05). The cases with heart incident in elevation group ( 8 cases,19.51%) were significantly more than in no drift group ( 4 cases ,10.81%) (P<0.05). Conclusions ST segment elevation of aVR lead in patients with acute anterior wall myocardial infarction is useful for predicting prognosis of acute anterior wall myocardial infarction.
