胸腹部孤立性纤维瘤的CT 诊断及鉴别

目的：探讨胸腹部孤立性纤维瘤（SFT ）的CT表现，提高对本病的认识。方法回顾性分析经手术病理证实的5例SFT的CT特征。结果5例患者中，胸部3例（肺内1例，胸膜1例，前纵隔1例），腹部2例（腹腔1例，腹膜后1例）。肿瘤边界清楚，形态不规则，4例呈分叶状，1例带蒂，最大径6．1～12．7cm ，平扫为软组织密度，2例可见钙化。增强扫描4例显著强化，强化幅度（45～90）HU ，瘤内可见增粗、迂曲血管影。结论胸腹部孤立性纤维瘤C T检查有一定的特征，部分腹部SFT病例可由腹主动脉直接参与供血，这与生殖系统肿瘤多由生殖动脉供血有一定的鉴别价值。

CT diagnosis and differential diagnosis of solitary fibrous tumors in thorax and abdomen

Objective To investigate the CT performance of thoracic and abdominal solitary fibrous tumor （SFT ） and to improve the understanding of this disease .Methods We retrospectively analysed the CT features of 5 patients with SFT occurred in thorx and abdomen ,which were confirmed by operation and pathology .Results In these 5 cases ,1 case occurred in lung ,1 in pleura ,1 in the anterior mediastinum .Peritoneal cavity was found in 1 case and 1 case was in retroperitoneal site .All of the cases showed clear border ,irregular shape ,4 cases showed lobulated signs ,one case presented with pedicle .The maximum diameter was from 6 .1 cm to 12 .7 cm .CT plain scan was that 5 cases presented with homogeneous density ,2 cases with calcification .On enhanced CT ,4 cases showed heterogeneous and significantly enhancement ,enhanced amplitude with （45-90） HU ,thickening and tortuous vascular shadow were also seen inside tumor .Conclusion CT examination has certain characteristics of SFT .In some abdominal and pelvic SFT cases ,when the abdominal aorta participates in blood supply to the tumor ,it’s also helpful to differential diagnosis ofreproductive system tumors .
