乳腺肿瘤中CerbB-2基因的表达与CPA血流的临床价值及相关性分析

摘    要:目的 探讨乳腺肿瘤中CerbB-2基因表达与彩色血管能量成像(CPA)的诊断价值及其相关性。方法 术前应用超声CPA多普勒技术常规检测49例乳腺癌患者及30例乳腺纤维腺瘤患者的肿瘤血流信号,与免疫组化法检测CerbB-2的基因表达情况做对照,并分析其相关性。结果 49例乳腺癌CerbB-2基因表达阳性率(71%)高于乳腺纤维腺瘤组(33.3%,P〈0.05),CerbB-2在临床Ⅲ、Ⅳ期的阳性表达率(83.3%)显著高于其在Ⅰ、Ⅱ期的表达率(50%,P〈0.01),伴有淋巴转移者CerbB-2基因的表达率为86.7%,无淋巴结者阳性表达率为45%,两者比较有统计学意义(P〈0.01)。CerbB-2的表达强度与CPA血流分级呈正相关(γ=0.683,P〈0.01)。结论 CerbB-2基因在乳腺癌中呈高强度表达,且与CPA血流的分级呈正相关,两者联合评价乳腺肿瘤能为临床早期诊断和预后评估提供重要信息。

The correlation of the CPA of ultrasound with the expressions of cerbB-2 and its clinical value in breast tumor

Abstract:Objective To analyse the correlation of the CPA of ultrasound with the expression of CerbB-2 and its clinical value in breast tumor. Methods Preoperatively, the blood flow signal of 49 patients with breast carcinoma and 30 cases with breast fibro-adenoma were measured by CPA. The correlation of the expressions of their CerbB-2 gene were detected with immunohistochemistry post-operation. Results The positive expression rate (71 ~) of CerbB-2 of the breast carcino- ma was higher than that of breast fibro-adenoma (33.5% ). The CerbB-2 positive expression rate (83.3 %) of breast carcinoma in clinical stages Ⅲ to Ⅳ, was significantly higher than that of the stages of Ⅰ to Ⅱ (50%). The positive rate of breast carcinoma with lymph nodes metastasis was 86.7 ~/00, while 45 ~/00 was found in non-metastasis. The comparison between these two groups revealed a significant difference. The expressive intensity of CerbB-2 had a positive correlation to the degrees of CPA flow ( γ = 0. 683, P 〈 0.01). Conclusion The high expressions of CerbB-2 in breast carcinoma were positively correlated to the CPA flow degree. CerbB-2 and CPA may provide important information for the earlier diagnosis and judgment.
